Summary

VA San Diego Healthcare System is currently recruiting for a full time Outpatient Mental Health Care RN to work in their Mental Health Outpatient La Jolla MH Clinics

  • Impact Unit#5. The Outpatient Mental Health Care Coordinator is a professional registered nurse who functions both in a clinical and coordinator role for the la Jolla MH Outpatient Clinics

  • IU#5.
    Responsibilities

The incumbent combines direct clinic nursing and case management skills, to support unmet, high priority needs for patients with mental health (MH) disease followed by the Impact Unit (IU) section. The Care Coordinator 1) provides continuity of care for MH patients followed by the IU section; 2) provides a safe and supportive environment for patients and their families; 3) incorporates the age, gender, culture and learning needs of patients requiring planning, coordination and evaluation of care; 4) applies infection control measures appropriately in delivering care; 5) adheres to ethical principles and nursing standards in delivering care; 6) uses skills for effective communication and patient education with individuals and groups to achieve desired patient outcomes; 7) possesses BLS certification; Psychiatric Mental Health Board Certification preferred; 8) Perform other duties as assigned by the IU Directors or Nurse Manager.
Other duties include but not limited to:
-Demonstrates sound clinical judgment and leadership in improving patient care through the use of the nursing process for patients.

  • Addresses problem-solving at the unit/team level, effectiveness in unit leadership role and as a consultant in the application of critical-thinking skills in documentation.

  • Provides age-specific care for adult, older adult (65-74), and geriatric care (over 75) patient/groups, including the following elements related to the aging process: promotes independence, encourages self-care, involves family or caregiver with care, provides necessary safety measures, assists patients to cope with and/or compensate for changes affecting mobility, balance, cognition, vision, hearing, bones, skin, and thermoregulation.

  • Utilizes interventions, which maximize health capabilities (i.e., safely administers medications, assists with coordination of consults, utilizes interdisciplinary team to achieve treatment goals).
    Work Schedule: Full Time, 40 hrs a week. OT/Shift/Weekends/Holidays may be required.

Qualifications

Basic Requirements:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing or The Commission on Collegiate Nursing Education . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. (NOTE: Applicant's education program must be listed on one of the above websites (CCNE or ACEN) showing accreditation at the time the degree was completed. If the school is not currently listed, applicants must provide in the application materials a written statement/evidence of past accreditation from ACEN or CCNE during the time they completed their program ) OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: BSN preferred. Certification in Psychiatric Mental Health Nursing and/or Case Management. Minimum of 2 years recent (within the last 3 years) RN experience that includes psychiatric experience. Demonstrates strong communication and customer service skills. This position will be located in the Mental Health Clinic in the La Jolla location.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I

  • An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
    Nurse I Level II

  • An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
    Nurse I Level III

  • An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    Nurse II

  • A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
    Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

Physical Requirements:
Heavy Lifting and Carrying (45 lbs and over); Straight Pulling, Pushing, Walking, Standing, Kneeling, Repeated bending, Keyboarding, Viewing computer screen (8 - 12 hours); Reaching above shoulder; Use of fingers; Both hands and legs required; Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16" to Jaeger 1 to 4; Far vision correctable in one eye to 20/20 and 20/40 in the other; Depth perception; Ability to distinguish basic colors and shades of colors; Hearing (aid permitted); and Emotional/Mental stability.
Other Physical Demands: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.
