Texas Health Allen is located at 1105 N Central Expressway, Allen, TX 75013
Salary Range: $26.50 - $50.40/hour, commensurate with working experience
Work Schedule: Full-time; 36 hrs./week; 7 AM - 7 PM, every other weekend required
Our 8-bed mixed ICU Department (Level IV trauma center) is in need of a Registered Nurse on a full-time basis. The ideal candidate will be a team player who is excellent at communicating with peers, physicians, patients and who cares about customer service

The ideal candidate will possess the following qualifications :
• Associate's degree in Nursing required;
• BSN preferred. Completion of BSN within two years of earliest start date in a Texas Health RN role is required
• One-year experience as a clinical nurse, or completion of an RN residency program.
• Current RN licensure or compact licensure recognized by the Texas Board of Nursing upon hire.
• Three (3) years recent ICU experience preferred. RN must practice within the regulations of the Texas Nurse Practice Act, serve as a role model, intra/interdisciplinary collaborator and coordinator for quality patient care
• RN - Registered Nurse Upon Hire Required; Current RN licensure or compact licensure recognized by the Texas Board of Nursing upon hire.
• BCLS - Basic Cardiac Life Support Upon Hire Required
• ACLS - Advanced Cardiac Life Support Upon Hire Required
• CPI - Crisis Prevention Intervention Training 60 Days Required
• CCRN (Critical Care Registered Nurse) Preferred

VIEW JOBS 10/4/2019 12:00:00 AM 2020-01-02T00:00 Overview Case Manager Registered Nurse (RN) PRN PAM Rehabilitation Hospital of Allen The Case Manager Registered Nurse (RN) is responsible for: * Directly contributing to the creation of an outcomes-based patient care delivery system that is a planned process involving service coordination, monitoring, and management to achieve quality clinical outcomes, patient/physician/payor satisfaction, and cost effective utilization of resources * Maintaining and enhancing payor relationships * Assesses, plans, monitors, and coordinates plan of care from pre-admission to community re-entry through the implementation of case management standards and process * Assesses the individual's personal and medical history, current status, diagnosis, prognosis, and the proposed treatment plan * Proactively communicates with payor source to explore coverage solutions (flex benefits, out of contract, etc.) * Utilizes rehabilitation/LTCH/SNF criteria to promote optimal outcomes for the patient within the boundaries of the diagnosis * Coordinates and facilitates treatment team meetings, plan of treatment and IRF- Patient Assessment Instrument/ MDS, as appropriate * Proactively collaborates with the health care team, payors, community agencies, providers and legal representatives to ensure continuity throughout the continuum * Serves as a liaison with the treatment team and the Primary Care Physician, referring physician, medical director, patient/caregiver(s), and other parties as appropriate Qualifications The Case Manager Registered Nurse (RN) must: * Hold current License as a Registered Nurse as required by the state in which the facility resides * Have 2-3 years of related experience to assess clinical and psychological needs and interface with provisional staff, both external and internal, preferably in a post-acute setting Post Acute Medical, LLC, (PAM) based in Enola, Pa., provides post-acute healthcare services through 30 long-term acute care hospitals and medical rehabilitation hospitals and 15 outpatient physical therapy locations in nine states.
