RN, Intra-Op

Assesses plans and organizes for the individualized intraoperative care of the patient.

1.1 The pre-op assessment is completed and fully documented in the nursing record.

1.2 Patients problems and needs based on the data obtained in the pre-op assessment are identified and documented.

1.3 Plans and prepares for the care of the patient according to diagnosis, procedure, needs of the patient, and the physicians preferences, using preference card, prior to scheduled procedure time.

1.4 A written plan of care is developed based on identified patient problems and needs.

1.5 All equipment is checked for proper functioning before each use as evidenced by lack of problems.

1.6 Supervise personnel team actions to promote readiness and timeliness for surgical procedures.

Implements nursing care assuring patient's dignity and safety.

2.1 There is documentation that the care given is in accordance with the written plan and complies with patient care standards as defined in department standards of care.

2.2 Patient safety is provided according to department policy.

2.3 The patients comfort, dignity, and psychosocial needs are met according to established standards of care.

2.4 Family concerns are met by notifying them of surgery delays and by periodic reports of progress as evidenced by nursing record documentation.

Effectiveness of services provided is evaluated.

3.1 Assess and document the patient's response to nursing interventions.

3.2 Document and verbally communicate patient assessment to appropriate personnel upon transfer to another area.

3.3 Uses supplies and equipment in an economical manner, using physician preference cards and nursing judgment, without sacrificing the care of the patient according to Mercy philosophy.

3.4 Patient charges are documented with 95% accuracy rate.

Assists in the care of equipment and supplies to insure availability when needed.

4.1 Cleans the room and carefully removes equipment, according to infection control procedure and to insure safety after the procedure is terminated.

4.2 Assists and knows procedure for cleaning for cleaning, sterilizing, and storing instruments and equipment, according to procedure to insure availability and safety.

4.3 Assists with inventory once a year as assigned and daily by reporting and documenting use of supplies and malfunctioning equipment, upon discovery, to insure availability when needed.

Department procedures are followed for the protection of patient and staff.

5.1 Proper OR attire is worn at all times according to written policy.

5.2 Principles of aseptic technique are adhered to at all times.

5.3 Staff members follow established procedure for surgical hand scrub.

5.4 Universal precautions and infection control guidelines are followed.

5.5 Written procedures for care and handling of surgical specimens shall be followed.

Maintains and increases standards of performance and education and aids in self and staff development to insure excellence.

6.1 Attends departmental huddles.

6.2 Assists with orientation of new employees by sharing her/his knowledge and talents.

6.3 ACLS and CPR is current

6.4 Attends all mandatory in-services, hospital or department

6.5 Exhibit willingness to be flexible in shifts worked. Willingly stays late as needed and returns to work if called and not on call.

Demonstrates knowledge of special needs and behaviors specific to age group.

10.1 Demonstrates knowledge of the changes associated with development in the pediatric patient.

10.2 Demonstrates the ability to provide care based on physical, psycho-social, cultural, safety and other age related factors.

10.3 Demonstrates the ability to obtain and interpret information in terms of patient's needs.

RN, Intra-Op:

Education: Graduate of accredited School of Nursing.

Licensure: Current licensure to practice nursing in the state of Oklahoma.

Certifications: BLS certification. ACLS & PALS certification within first year of employment.

Preferred Experience: 3 or more years of intra-op RN experience.

Mercy has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

RN, Pre-op & Post-op: Assists in the planning and provision of nursing care. 1.1 Plans and prepares for the care of patients according to diagnosis, procedure and the needs of the patient. Will work in all areas of the department. 1.2 Assures that all procedures start in a timely manner by carrying out all preoperative orders, starting the IV, reviewing and posting laboratory reports. 1.3 Keeps Anesthesia and Circulator informed of any pending delays. 1.4 Introduces themselves to the patient and families in a warm manner. The nurse assists them in completing forms, signing surgical permits. 1.5 Starts the IV with 97% first attempt success rate. 1.6 Explains the routine of the day to the patient and visitors. Instructs the visitors on where the waiting room is located. 1.7 Meets family/visitors concerns by notifying them of delays or changes that may occur, evidenced by documentation in the nursing record. 1.8 Assesses and documents patient's response to nursing interventions following procedures. Receives and/or provides timely report on any changes to appropriate person for continuity care. 1.9 Accurately documents and verbally communicates patient assessment to appropriate person upon transfer to another care area. 1.10 Documents economical use of supplies in all areas based on the patient's physical needs. Completes unit activities as assigned. 2.1 Routine and emergency equipment is checked for proper supply and function according to established procedures. 2.2 Completes Post-operative call backs as assigned. Implements nursing care assuring the patient's safety and dignity. 3.1 Initiates written and or verbal orders. 3.2 Patient safety is provided according to department policy. 3.3 The patient's comfort, physical needs, and dignity are met in accordance with established standards of care. 3.4 Responds appropriately to irregularities and/or emergency situations through initiation of ACLS/PALS protocols and prompt communication to anesthesia. 3.5 Assess and document the patient's condition immediately on admission and to be repeated as warranted. 3.6 Demonstrates the ability and willingness to care for patients at the nationally established nurse-patient ratio based on acuity. 3.7 Discharges patients per orders in a timely manner. Effectiveness of services is evaluated. 4.1 Patient charges are documented with 95% accuracy rate. 4.2 All records and medication sheets are completed accurately and timely as evidenced by documentation. 4.3 All pertinent carbons or duplicate records are removed and filed correctly. 4.4 Discharge and transport policies are followed 100% of the time in all areas. Any variations have a written physician's order. Provides patient care in the Phase 2 areas. 5.1 Initiates post procedure patient teaching as soon as the patient or family/responsible agent is able to understand. 5.2 Prepares patient for discharge by instructing the patient or family/responsible agent regarding home care of patient according to the physician's orders both verbally and in writing or printed format prior to discharge. 5.3 Documents patients/family or responsible agents understanding of discharge instructions. Assists in the care of the environment, equipment, and supplies to ensure comfort safety and availability according to Mercy Service Standards, Values and Charisms. 6.1 Maintains clean and neat area and equipment at all times to ensure safety of patients, visitors and personnel according to safety standards and hospital policy. 6.2 Assists with inventory of daily supplies needed for smooth operation of department by the end of each shift. Restock rooms and care area daily and as needed. Department policies and procedures are followed for the protection and safety of the patients and healthcare workers. 7.1 Adheres to dress code polices. 7.2 Sterile technique principals followed and practiced at all times. 7.3 Universal precautions and infection control guidelines are followed and practices at all times. 7.4 Emergency call procedures are followed at all times 7.5 Contributes to the smooth functioning of the department by meeting department requirements, such as, willingly working flexible shifts, staying late as needed, and returning to work from home as needed even if not on call. Participation in organizational activities. 8.1 Attends all mandatory in-services, hospital or department. 8.2 Maintains current RN licensure, and certification in CPR, PALS, and ACLS. The nurse will exhibit the following for professional development. 9.1 Assists in orientation of new personnel or students. 9.2 Exhibit willingness to be flexible in shifts worked.
