RN Case Manager

The RN Case Manager is responsible for the assessment, planning, and coordination and provision of direct hospice care services to the hospice patient and family unit in accordance with the written hospice patients plan of care.


  • Must have a current RN License in the state of Arizona.
  • Have at least one-year experience in hospice
  • Have the ability to communicate well
  • Have knowledge/ Proficiency in computers

Work Hours:

  • Full Time employee with assigned on-call responsibilities.

Specific Job Duties:

  • Evaluate the effectiveness of the hospice services to the patient and family on an ongoing basis.
  • Conduct initial as well as ongoing comprehensive assessments of the patient in a complete and timely manner.
  • Evaluate the patients condition and provide care concerning changes in patients condition requiring order changes and/or changes in treatment.
  • Participate in IDG and case conferences.
  • Designated to ensure continuous assessment of the patient and familys needs and to coordinate activities with other members of the IDG.
  • Provides education to patients family members regarding the dying process, and the plan of care and offers guidance with healthcare decisions.
  • Documents all services rendered to the patient by the Hospice and all changes that have been noted in the patients condition and/or family and home situation and makes revisions to care plan as needed.
  • Maintains and updates his or her knowledge base to ensure updated and current knowledge of hospice and palliative care.
