Provides quality care to patients in their homes under the direction of a physician. The nurse evaluates safety concerns and coordinates home health care services that reflect the home health care agency philosophy and standards.

Hiring Competencies

a. Education: Bachelor's degree in Public Health or Nursing preferred. A copy of your highest academic diploma or transcript required upon hire.

b. License: Current license to practice professional nursing in the state of Oregon required.

c. Certification: American Heart Association Healthcare Provider BLS required upon hire.

d. Current Oregon or Washington driver's license, be at least 21 years old with minimum of two years' experience in the type of vehicle being driven. No more than one accident and two moving violations or two accidents and one moving violation or no accidents and three moving violations on or off the job within the past 36 months and no conviction for driving under the influence of intoxicants (DUII), license suspension, auto felony conviction or other serious violations within the past 36 months.

e. Clearance of a criminal background check and sex offender check is required every 3 years of employment as required by OAR 333-027-0064.

f. One-year experience in hospital medical/surgical nursing preferred.

g. One-year experience in home health care or public health nursing preferred.

h. Experience in physical assessments of geriatric patients preferred.

i. Critical care, oncology, or intravenous skills preferred.

j. Excellent customer service skills.

k. Ability to work without direct supervision required.

l. Excellent communication skills.

m. Knowledge of nursing principles and practices required.

n. Knowledge of the nursing process and its applications to patient care required.

o. Provides care appropriate to the age of patients served on the job.

p. Must be able to develop skills in use of computerized information/Allscripts system used in home care setting.

q. Excellent documentation skills required.

r. Professional personal appearance makes a good first impression.

s. Bilingual skills a plus.

Promotes a positive physical and psycho social environment for the residents. Essential Functions: * Plans and directs ongoing care in collaboration with shift supervisor, recognizing individual needs of residents and staff. * Monitors work assignments, provides feedback, evaluates performance, redirects staff, and informs Director of Nursing Services of pertinent problems timely. * Communicates and coaches staff regarding policies, completes quality care audits, and supervises professional staff in the daily deliver of resident care. * Assist with new admissions, transfers, and discharges ensuring related documentation is current, signed, and the record keeping meets the policies and procedures. * Reviews and completes resident records and related documentation in a timely manner, including the completion of nursing assessment for each resident within 14 days of admission. * Plans and conducts resident care conferences and develops interdisciplinary plan of care for each resident. * Regularly observes audits and assesses each resident's condition, status, and care requirements and takes appropriate action to assure care needs are met. * Develops nursing care plans for each resident that meets the needs of the resident and the physician recommendations and reports changes in condition of residents that requires nursing intervention notifying the physician, director of nursing services, and family. * Transcribes and carries out or delegates physician orders in accordance with the nurse practice act. * Documents all pertinent information in the interdisciplinary notes and on the 24-hour DNS report form timely and accurately. * Performs other related duties as assigned. Qualifications: * Must have a current, unencumbered Registered Nurse (RN) license. * Associate Degree in Nursing or Bachelor of Science in Nursing. * Minimum of 1 year working in geriatric nursing, including experience with medication administration, delivery systems and pharmacies. * One year in supervisory/management position, with experience with budgets, staff development, training and scheduling. * Additional course work in management an asset. * Excellent written and oral communication and motivational skills are essential to success. * Ability to creatively problem solve in both resident care and employee management situations. * Individual must be a team player, well organized, and flexible.
