Your responsibilities:

  • Assess home care patients identifying physical, psychosocial and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, call-in logs and on-site evaluations

  • Complete OASIS, assessment and visit paperwork according to agency policy

  • Communicate significant findings, problems and changes to Manager, Clinical Services and physician, and documents all findings, communications, and appropriate interventions

  • Participate in interdisciplinary patient care conferences and Agency Performance Improvement processes

  • Assure the continuity of patient care and coordination of services through delivery of quality patient care

Required skills and qualifications:

  • Current license as a Registered Nurse in good standing for the state

  • 1-2 years of prior home health experience is needed

  • Home Care Home Base exp is desired

  • One year of home health, public nursing or acute hospital nursing experience

  • Familiar with Medicare home health regulations, documentation requirements, ICD-10 coding and PPS (Strongly preferred)

  • Acute care and home care experience are preferred

  • Must have valid driver's license

Position Summary The Float Pool Registered Nurse provides direct bedside care on Progressive Care Unit, Medical/Surgical Unit, Interventional Cardiac Care Unit, Emergency Department Holds or Clinical Decision Unit. Essential Functions * Assess patient's needs and develops/revises individualized plan of care based on patient needs and responses. * Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served * Knowledgeable about the principles of growth and development over the life span, assessing the data reflective of the patients' status and interpreting the necessary information needed to identify each patient's requirements relative to individual age specific needs * Differentiates severity of patient problems and prioritizes care within the patient assignment, establishing an acuity level consistent with patient assessment and nursing diagnosis * Independently develops a nursing plan of care based on established standards of current evidence based practice * Initiates nursing interventions and protocols to ensure desired patient outcome is achieved * Evaluates and re-evaluates patient's plan of care and response to illness, treatments and nursing interventions throughout the patients' stay. Makes appropriate revisions to achieve desired patient outcomes. * Utilizes critical thinking to recognize changes in the patient's condition and communicates significant findings to the appropriate discipline * Collaborates with interdisciplinary team to ensure timely and appropriate care occurs that facilitates effective patient throughput * Demonstrates ability to set priorities and make appropriate decisions when dealing with changing patient conditions and census. Works efficiently to maintain continuous unit functions. * Evaluates patients learning style and provides patient and family education accordingly. Utilizes teach back method to evaluate patient/family understanding of teaching. * Documentation is accurate, consistent, concise, timely and complete reflecting patient responses and outcomes * Effectively communicates appropriate discharge instructions and referral for follow up care * Assists in obtaining current medication list upon admission. Utilizes appropriate technology as applicable to ensure medication administration is timely and complete using established safety mechanisms * Independently identifies the need for and demonstrates accurate and timely hand off communication * Able to mobilize resources to respond to ethical and cultural concerns. * Develops therapeutic relationships through presence with patients/families and active listening * Assumes responsibility and accountability for nursing care when delegating tasks and determines the appropriateness of the delegated task based on American Nurses Association principles of delegation and state regulatory requirements.
