Case Management and Supervision

  • Follows VITAS policies and standards (See VITAS Standards Manual) and practices within the scope of the RN license

  • Reports any request to practice outside of scope of license to the manager/supervisor

  • Supervision of (VITAS employed and contracted) nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses (and contracted Registered Nurses) and Volunteers per the plan of care

  • Completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement

  • Appropriately identifies/utilizes resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults

  • Completes documentation at the time of the event and at the location of care unless otherwise directed

  • Provides education to patients, families, caregivers and referral sources

  • Participates in patient care meetings as assigned

  • Participates in weekly team meetings

  • Provides effective communication/shift reports through written, verbal, reporting and body language

  • Participates in bereavement support

  • Participates in the on-call rotation and/or provides shifts during non-business hours such as evenings, weekends and holidays based upon program-identified needs

  • Provides visits/shifts in program coverage area

  • Develops, coordinates, and follows the patient's individualized plan of care

  • Appropriately delegates nursing tasks to Hospice Aides/Homemakers, Volunteers, and Licensed Practical/Vocational Nurses

  • All factors pertinent to the patient's safety, including the competence of the Hospice Aide/Homemaker have been assessed

  • Determination that this activity can be delegated safely to a Hospice Aide/Homemaker

  • Connecticut -For activities other than those listed in section 1913D69(d)(3)of the Connecticut Department of Public Health delegated to a Hospice/Homemaker, the RN will document in the clinical record that:

  • Provides nursing care including nursing tasks that may be delegated to other roles such as LPNs, HHAs and Volunteers

  • Participates in and monitors patient and family care compliance activities

  • Professionally manages/coordinates the hospice plan of care

  • Provides and follows infection control practices

  • Delivers care in accordance with the patient's plan of care

  • Provides primary nursing care for inpatient and continuous care patients as assigned

Quality Improvement

  • Participates in the Quality Assessment and Performance Improvement (QAPI) Program and related activities as assigned. This may include, but is not limited to
  • participation in Performance Improvement Project(s) (PIP)
  • Gathering clinical data which is used for quality measures

Professional Development

  • Develops and achieves professional growth goals and objectives personally and per Team Manager reviews

  • Responsible for attending orientation and regularly scheduled in-services/educational conferences as assigned

  • Participates in orientation of Hospice Aides, Volunteers and other IDG members as assigned

  • Participates in training activities internally within VITAS and externally as assigned

Customer Service

  • Maintains focus on the patient and family experience of care

  • Serves as a patient and family advocate

  • Reports patient/family/customer complaints to Team Manager

  • Participates in patient/family/customer conflict resolution as applicable.

  • Represents the nursing profession, the VITAS site/location and VITAS overall in a positive manner

QUALIFICATIONS

  • Currently licensed as a R.N. in the state where the VITAS program is located.
  • A minimum of two years experience in hospice, home health, or community health experience in the last five years.

  • RN license

COMPETITIVE PAY RATE PER VISIT. Essential Functions: Participates in ongoing interdisciplinary assessment of the patient. Completes initial and ongoing comprehensive assessments of the patient's needs at appropriate time points as assigned. Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s). Contributes to the patient's plan of care/service plan. Participates with the patient in clarifying mutually agreed upon goals and incorporates these goals into the plan of care/service plan. Provides services that are ordered by the physician as indicated in the plan of care. Counsels and educates the patient and family in meeting healthcare related needs. Initiates appropriate preventative and rehabilitative nursing procedures. Completes timely and accurate clinical notes including addressing the patient's progress. Refers to other services as needed. Monitors and evaluates outcomes of patient care. Communicates with appropriate staff regarding changes in the patient's needs. Assigns home care aide to a specific patient. Supervises paraprofessional staff. Coordinates plans for patient discharge from services. Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Follows accepted standards of nursing practice. Uses clinical decision making to efficiently and effectively manage individual patients. Participates in Quality Assessment and Performance Improvement (QAPI) program. Participates in in-services or training, as requested. Participates in the Office's Emergency Preparedness program, as requested. Contributes to the effective implementation of company programs and services. Contributes to Office business growth. Provides other assignments as requested and within the scope of licensure. Minimum Education & Experience Requirements: Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the state(s) in which he/she practices. At least 1200 hours of experience as an RN or completion of the Office's preceptor program. Knowledge, Skills & Abilities Required: Able to perform and provide evidence of current CPR certification for adults, children and infants. Able to demonstrate basic understanding of computers and workplace software. Able to work with people, function well under stress, use clinical judgment, be creative and accept challenges. Able to effectively communicate in English with all levels of the workforce, both clinical and non-clinical. Able to perform and prioritize multiple functions or tasks. Able to read and interpret technical instructions related to the care of the patient. Able to visually and aurally observe and assess the patient. Able to effectively deal with multiple changes based on patient needs and scheduling. Able to provide proof of valid driver's license, if applicable. Able to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle to transport patient. Working Conditions & Physical Effort: Able to constantly travel within geographic area serviced by office from assignment to assignment. Meets the applicable health requirements to provide patient care per applicable law or regulation. Work is normally performed in patient's home, schools, or occupational settings. Frequent exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions and may include the use of protective equipment. Able to constantly stand, walk, bend, stoop, squat, kneel and reach freely. Able to frequently lift, reposition, and transfer patients. Able to constantly grasp with thumb in opposition to fingers of palm, i.e. manipulate pen, knobs or objects.
