Summary

The RN Visiting Care Coordinator is responsible for planning, coordinating, and ensuring continuity of patient care, while maintaining strong relationships with the patient, family, and physician from admission through discharge. Key responsibilities include patient advocacy, clinical competence, and effective resource management.

Schedule

• Fulltime | 64 hours per two weeks

• Daytime shifts | Mon-Fri 8:00AM to 4:30PM| Call shifts required every 5th weekend | Min. 2 shifts per 4 weeks | Some holiday hours call shifts possible

Qualifications

• Current MN Registered Nurse License.

• Driver's License required.

• Reliable transportation to make client home visits.
