Job Description Job Description Staff Requirement (all direct service staff) · Must be 18 years of age · All staff must show proof of identification, current license, high school diploma or equivalent, & degree obtained · Must pass criminal background check, EMR (Employee Misconduct Registry) & NAR (Nurse Aide Registry) · Must have current CPR & first aid certificate · Must have TB test done with- in 2 weeks of employment and annually Nurse/LVN Requirements (Full-Time or Part -Time) Must be licensed from the Texas Board of Nursing and practice in compliance with the Nurse Practice Act and rules and regulations of the Texas Board of Nursing · Must have high school diploma or equivalent Current CPR certified, negative TB test · Two years of full- time work experience in the nursing field working with people with disabilities in a human service or medically related program Must be available 8 hours Monday – Friday (7am – 5pm) If an LVN is employed as the facility nurse, then an RN consultant must provide consultation at the facility at least 4 hours per week. The RN consultant must document the consultation provided, and must be provided when individuals are present in the facility Nurse/LVN Duties · Initial Assessment within 14 days of the start date to determine type of care needed · Obtain physicians order for medication & treatment to be administered · Develop individual’s plan of care according to physician order & participant needs · Determine whether self-administered medications can be done and have been appropriately taken, applied, or used · Document monthly progress on medical care provided with dates · Train, consult, and assist the LVN to maintain proper medical records and · Provide health education to participant & direct service staff · Maintain medical records · Assess & chart observation of participant behavior, condition, and care rendered daily · Check vitals, weight, blood pressure and medication monthly for review of care plan · Counsel & educate participant & family about their plan of care · Recommend tools or devices that may improve the quality of life for participant · Collect patient health history & make treatment decisions when appropriate · Assist with field trips & outings · Duties may changes as needed · 3 hours of ongoing training quarterly · Monthly evacuation procedures practice · Fire, disaster, & evacuation training within 3 working days and · 18 hours of training during the first three months · Maintain confidentiality Key Skills · The ability to communicate in a clear and understandable way · The ability to interact well with people from a wide range of backgrounds · Provide Sensitivity & Empathy when needed · Have a passion for helping others, and demonstrate excellent social skills
