Under the general supervision of the Director of Emergency Services, is responsible for safe, effective and quality patient and family centered care in the Emergency Department.

This is a per diem position.

1 Plans, evaluates, directs and renders care according to established standards of care
2 Incorporates evidenced based best practice into everyday clinical activities
3 Performs medication reconciliation and administers medications safely and appropriately using the 5 rights of medication administration, leveraging technology and pharmacy resources
4 Recognizes and performs nursing interventions to maintain the body's homeostasis (acid-base balance, fluids and electrolyte imbalances, wound and tissue perfusion)
5 In accordance with standing Emergency Department medical protocols and standing orders; enters orders, interprets and evaluates diagnostic tests to identify and assess patient's condition, communicates same treating provider
6 Maintains accurate, detailed clinical nursing documentation, reports and records
7 Within the scope of nursing practice, provides oversight of nursing care activates of ancillary nursing/healthcare teams member
8 Demonstrate flexibility, practice and collaboration to cross train in other departments
9 Consistently utilizes clear, concise, professional communication in all interactions including; with patients, families, between disciplines, inter-departmentally and with outside agencies

  • Education: Graduate from an accredited nursing program, BSN preferred

  • Experience: One (1) year of Emergency Nursing experience preferred. Must be computer literate and fluent in electronic health record documentation

  • ACLS

  • BLS

  • TNCC within 1 year of hire

Provides patient education and coordinates treatment with Palliative Care Team and agencies for the delivery of consultative palliative care services. PRINCIPAL DUTIES AND RESPONSIBILITIES: Within established policies and procedures, and consistent with the CMCM Medical Staff Bylaws, rules and regulations, the incumbent performs the following functions: Primary Functions Interviews patients and/or families to obtain comprehensive history and data about patient's physical and developmental status and behavior. Completes a physical and psychological assessment based on interview, examination and available data. Identifies medical and health issues and needs based upon the evaluation. Formulates differential diagnosis based on history, physical and clinical findings. Reviews clinical data from lab results, x-rays, scans and pathology reports. Determines clinical appropriateness of team member who will see patient in consultation. Develop a treatment plan which may include diagnostic testing, pharmaceutical and non-pharmaceutical interventions as well as education. Coordinate treatment and review with collaborating Medical Director, Patient's attending physician and the Palliative Care Team. Documents data and encounters with the patient and family in the Electronic Medical Record (EMR). Follows progress of patient care to determine the effectiveness of the treatment plan. Updates and records changes in health status. Modifies treatment to achieve clinical goals. Identifies patient and family learning needs, the need for multi-disciplinary resources and provides instructions and patient education. Adjusts content based on patient's readiness to learn in the time allowed. Validates patient learning and documents level of understanding and ability to perform. Writes orders for prescriptions, nurse dispensed medications, diagnostic testing and consults as appropriate. Adheres to infection control procedures, safety procedures and Universal Precautions. Ensures confidentiality of employee, legal, client/patient, budget and all company matters. Demonstrates courtesy, respect and caring to patients, families, physicians and coworkers and maintains professional rapport with external parties and organizations. Maintains documentation in the Electronic Medical Records in accordance with CMC policies, requirements and standards. Participates in Palliative Care Committee and program meetings and cooperates with physicians and coworkers to achieve program objectives. Reports concerns and problems as they arise. In collaboration with Palliative Care Committee, reviews data and analyzes PI activities to improve quality of care for Palliative Care Program. Provides and promotes effective and compassionate care for patients and their families. Develops educational programs for patient, providers, staff and community education including educational materials. May represent Palliative Care on various hospital committees as appropriate. Secondary Functions: Participates in continuing development activities, maintains professional affiliations and licenses and strives to increase skill and knowledge. Attends seminars and workshops for professional growth and development. Participates in clinical meetings of the practice to develop a team approach to peer support and development. Participates in appropriate role in disaster, fire drills, fires and other emergency situations. Exercises care in the operation and use of equipment and reference materials. Maintains work area in a clean and organized manner. Performs similar or related duties as assigned or directed. Ability to transport self to and from off-site locations. OSHA RATING: Category I (Exposure to blood borne pathogens) PHI ACCESS: Complete Education: Graduate of an accredited APRN program. Experience: Five years or more as a registered nurse with strong medical/surgical background. Two years as a practicing adult nurse practitioner preferred.
