Purpose of Position: Functioning as a member of a team, the registered nurse in collaboration with MAT prescribers and multidisciplinary team members, is responsible for coordinating, facilitating the clinical management of our Medically Assisted Treatment (MAT) patients throughout the continuum of care contributing to optimal patient outcomes. Responsibilities: Initiates and completes patient assessments throughout the continuum of care; identifying physical, social and psychological response of patient and family using interviewing and physical assessment skills. Use professional nursing judgment regarding patient#s condition in relation to physician#s orders and protocols and administering medications and treatments, optimally upholding a safe patient environment and clinical practices. Demonstrates the ability to establish therapeutic relationships with both adult and adolescent patients. Demonstrates understanding of group dynamics and effectively utilizes the group process to meet the psychological needs of adult and adolescent populations. Implement plan of care utilizing nursing interventions which reflect an understanding of the nursing process, standards of practice and standards of care. In collaboration with the health care team, assess and identify high risk patients requiring on-going coordination of care and develop a comprehensive plan of care that will span the continuum. Receive, handle, store, administer, and dispose of medications according to policy and standards to optimally limit unnecessary access and secure medications throughout these processes. Administer intravenous, subcutaneous, and intramuscular therapies in accordance with department scope of service and Medical Center and department policies. Responsible and accountable for own patient assignment and for that of co-assigned RN/LPN/LNA/Student Nurse, appropriately delegating and supervising unlicensed and licensed assistive personnel and licensed practical nurses in accordance with the New Hampshire Nurse Practice Act and professional standards of nursing practice. Works independently as well as collaboratively in order to optimize patient care and patient experience. Function within the boundaries of hospital policy and safety, infection control procedures and standards and the Scope of Nursing Practice as defined by the New Hampshire Nurse Practice Act, and with respect for Patient#s Bill of Rights. Is accountable for appropriate use of materials and resources in planning and delivering care. Minimum Qualifications: Graduate of accredited nursing program. BSN preferred. At least two years of RN experience working with the public. Ability to handle confidential material with maturity, sensitivity and discretion including 42 CFR Part 2 adherence and HIPPA compliance. Excellent interpersonal and communication skills.

Responsibilities: A listing of the key responsibilities

1.Initiates and completes patient assessments throughout the continuum of care; identifying physical, social and psychological response of patient and family using interviewing and physical assessment skills.

2.Use professional nursing judgment regarding patient's condition in relation to physician's orders and protocols and administering medications and treatments, optimally upholding a safe patient environment and clinical practices.

3.Demonstrates the ability to establish therapeutic relationships with both adult and adolescent patients.

4.Demonstrates understanding of group dynamics and effectively utilizes the group process to meet the psychological needs of adult and adolescent populations.

5.Implement plan of care utilizing nursing interventions which reflect an understanding of the nursing process, standards of practice and standards of care.

6.In collaboration with the health care team, assess and identify high risk patients requiring on-going coordination of care and develop a comprehensive plan of care that will span the continuum.

7.Contribute to the development and maintenance of a care delivery system which is sensitive to patient needs and expectations, promotes effective utilization of resources, and supports physician practice, while emphasizing care coordination across the continuum.

8.Communicates with payer liaison and third parties as necessary to determine appropriateness of admission type and setting; obtain prior authorizations; provide concurrent clinical information; investigate denials, and respond to denials and pursue appeals.

9.Provide individualized patient care in a manner which demonstrates clinical knowledge, MAT specific interventions, technical competence and knowledge of patient population, age-specific behavior, growth and development for scope of service; documenting information relevant to patient status and response to care.

  7. Implements a goal directed, age-appropriate plan of care with coordination of the health care team, reassesses and revises the plan as needed.

  8. Treat each patient, family, visitor and employee with compassion and courtesy; maintaining excellent customer relations, a positive attitude, and an ability to work with others in a cooperative effort to ensure customer satisfaction and Continuous Quality Improvement.

  9. Participate in the development and application of interdisciplinary evidence-based care paths/clinical protocols and facilitate the achievement of desired clinical and financial outcomes.

  10. Participates in staff and physician education, as appropriate, regarding core measures, clinical pathways, payer requirements, and managed care principles.

  11. Conducts necessary patient education regarding medications, procedures or other therapies being suggested, utilizing appropriate written instruction.

  12. Performs call back evaluations on patient receiving interventional therapies to assess outcomes.

  13. Demonstrates exceptional communication, conflict resolution and problem solving skills with patients/families, intervening to prevent or resolve patient/family concerns.

  14. Is accountable for JCAHO/NCQA standards and serves as departmental JCAHO compliance monitor.

  15. May be required to perform other duties of lesser, equal or greater levels of responsibility.


Minimum Qualifications:

ü Graduate of accredited nursing program. BSN preferred.

ü Excellent interpersonal and communication skills.

ü Cultural sensitivity as evidenced by an ability to work with marginalized populations.

ü Ability to function as a member of a multidisciplinary team, handling a variety of tasks and fluctuating workloads.

ü Ability to work independently and prioritize own work and the work of others.

ü Excellent problem solving skills.

ü Competency handling electronic information and functional knowledge using clinical applications.

Required Licensure/Certification Skills:

NH RN License. BLS within 3 months of hire and maintained on a bi-annual basis.
