DaVita is seeking a Registered Nurse who is looking to give life in an outpatient dialysis center. You can make an exceptional difference in the lives of our patients and their families dealing with end-stage renal failure or chronic kidney disease.
If you haven't considered Nephrology before, read on as we think that you should. Dialysis Experience is NOT required.

  • Long term patient relationships. Build meaningful relationship with patients and their family in an intimate outpatient setting.

  • A team that feels and functions like a family. Staff RNs are a central part of a small interdisciplinary team of clinicians. Work alongside and support technicians, other nurses, dietitians, social workers, physicians and the center manager.

  • Specialized, complex care. RNs deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.

  • Fast paced. Our RNs oversee a group of patients (census dependent on state laws) with the support of PCTs to conduct patient observations, measuring stats and machine set up. You will work with your head, heart and hands each day.

  • Schedule. Shifts are typically 10-12 hours, 3-4 days per week. Schedule times will vary by locations but most of our RNs work early morning or days shifts. Some holidays required.

  • Some details about this position:*

  • Training may take place in a facility or a training clinic with a class of new teammates other than your assigned home clinic

  • You must be flexible on the final home clinic placement it may be in a different location; there is also the potential to float to various clinics during and after your training

  • You must have a flexible schedule and be able to work mornings, evenings, weekends, and holiday

  • Current Registered Nurse (RN) license in the state of practice; Graduate Nurse pre-licensure available in some locations

  • A desire to deliver care in an empathetic, compassionate way. You know, the way you'd want your own care to be

  • Experience assessing, trouble shooting and making sound recommendations in stressful situations

  • Excellent communication skills to listen and communicate with patients and teammates

  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree

  • Basic computer skills and proficiency in MS Word and Outlook. You will be required to chart patient's assessment, orders and your care notes in our electronic medical record system.

  • You might also have (a.k.a. nice-to-haves):

  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience

  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)

Hospice Career FAQs Sign Up for Career News * Provide clinical assessment and intervention utilizing the nursing process, VITAS Palliative Care Guide, and Triage Protocols. * Serve as the patient and family advocate by communicating with the attending physicians, long-term care facility staff, case managers, and others external to VITAS as necessary. Provides collaboration with LPN/LVN's. * Coordinate all service delivery after hours including patient and family education, dispatching of interdisciplinary visits, and coordination of services with external vendors and resources as needed. * Provide support and collaboration with Telecare staff to ensure favorable patient outcomes. * Provide bereavement support to families. * Document interactions with patients, families, contacts, and vendors in the patient record to include the assessment, plan of care, caller agreement to the plan, any actions and interventions and the resolution of each patient/family interaction. * Review all entries in the record made by Telecare LPN/LVN's and Patient Care Coordinators and ensures compliance with documentation Standards * Participate in the orientation of new team members as assigned by the supervising manager. * Attend regularly scheduled in-services, staff meetings, and educational conferences. * Develop and achieve professional growth goals and objectives and reviews with supervising manager on a monthly basis. * Represent professional nursing and VITAS Healthcare in a positive manner. * Seek certification in the specialty of hospice nursing when qualified to do so. * Instruct and document appropriate use of medications, home medical equipment and supplies. * Appropriately utilize the resources of contract personnel such as Pharmacy, HME, agency staff, and transportation services. * Promote a customer service-oriented approach to care delivery. * Support and promote all Care Connection Center cultural platforms * Compliance with all departmental standards, policies, and procedures with training and education, management of the workload, disaster planning, attendance, adherence, and quality standards QUALIFICATIONS * A minimum of two years' experience in acute-care hospital nursing in either medical-surgical, oncology, home health, or emergency preferred * Two years of customer service/call center experience preferred * Eligible for licensure in other states VITAS Triage services are located. * Excellent verbal, written, and interpersonal communication skills, as well as demonstrated effective telephone skills. * Ability to work as a team player and multi-task * Proficient in customer conflict resolution and crisis management. * Proficient in telephone techniques including phone etiquette, and handling of calls. * Thorough knowledge of professional nursing principles, methods and procedures; anatomy and physiology; medical supplies and equipment used in nursing practice; and the uses and effects of medications and narcotics. * Working knowledge of applicable state and federal laws and regulations pertaining to registered nursing and the scope of practice limitations of clinic support staff. * Ability to work weekends and holidays as necessary to support the operations of the Care Connection Center. * Ability to type 40 WPM EDUCATION * Associate's degree in Nursing or related field from an accredited college or university or the international equivalent required.
