General Position Description:
The Registered Nurse, Condition Management is responsible for improving the patient's overall chronic disease management and well-being. Acts as an advocate for patients and collaborates with physicians, advanced practitioners, clinical staff and community resources utilizing a proactive, multidisciplinary team based approach directed toward prevention, education and health promotion.

Principal Duties and Responsibilities:
Patient Care (75%):
Supports a proactive, multidisciplinary team approach directed toward prevention, education and health promotion.
Establishes supportive relationships with the patient and acts as a resource.
Assesses patient's educational/clinical needs, prioritizing and individualizing program based on elicited information.
Provides disease specific self-care education.
Collaborates with Physician or other Licensed Independent Practitioner and other Chronic Condition Management nurses to arrange for further medical treatment when needed.
Consults with other members of the healthcare team to solve problems and seek advice. Designs interventions to improve outcomes.
Assesses, screens, triages and performs treatments ordered for patients.
Identifies emergency situations and provides care within legal scope of practice.
Participates in outreach/follow-up phone calls and follow-up patient visits.
Assists in the development of criteria to measure outcomes, assists in the analysis of data for quality improvement, interprets study results, identifies limitations and barriers and recommends alternatives.
Develops and presents educational services for patients or departments as appropriate.
Delivers one on one and/or group coaching sessions in the areas of diabetes, asthma, weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general health.
May coordinate and conduct wellness programs in areas such as diabetes, nutrition, stress management, weight management, smoking cessation, exercise and others.
Works closely with third party health and wellness vendors.
Apply the use of logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions and approaches to problems.
Prioritizes and multitasks to optimize patient experience.
Ensures the well-being of patients and provides a positive, supportive environment for patients and other staff.
Completes patient referrals daily including referrals to client's other health and wellness programs.
Collaborates with the client's other in-house and third party health and wellness resources to provide an integrated experience for members.
Proper, concise and complete EMR documentation and paper based documentation for all assessments and procedures.
Comply with and follow policies and procedures as outlined by MedExpress.
Maintains HIPAA at all times.
Other duties as assigned such as new models of care and new types of procedures, etc.
Provides clear direction to achieve goals creating an environment that fosters team commitment and employee engagement.
Maintains the perseverance to drive and sustain the changes that occur at MedExpress while being resilient and flexible and inspiring and motivating the team.
Constantly communicates to the team in conjunction with the Lead Registered Nurse, Condition Management; is open to opinions and feedback from team members and follows through on commitments.
Creates a work environment in which people are able to perform to the best of their abilities.
May participate in obtaining and maintaining patient centered medical home accreditation to include, but not limited to, developing and implementing a quality improvement program, establishing and coordinating exchange of relevant patient information within the guidelines of HIPPA with local healthcare organizations and obtaining and analyzing performance data.
Participates in quality improvement projects to include the following topics: patient/primary care provider relationship, accessibility to care, comprehensiveness of care, continuity and/or coordination of care and clinical study.
Works with Center Managers and Lead Condition Management Registered Nurse to deliver team based approach to chronic disease management across the health care spectrum.
Learning and Ongoing Education (15%):
Maintain skills and competency for all chronic condition management.
Maintains technical skills and applies new knowledge to position according to MedExpress policy.
Maintains knowledge of Medical Terminology and standards of nursing care.
Maintains knowledge of common safety hazards and precautions to establish a safe work environment.
Reads and keep up-to-date on all ME Communications.
Completes all assigned training.
Center Readiness and Maintenance (10%):
Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA and Department of Health).
Maintains and complies with all MedExpress, Optum and Urgent Care Accreditation site assessment requirements.
Worksite clinical team members including providers and non-providers.
Worksite Administrative clinical partners.
External care services within vicinity of worksite.
Graduate of an accredited School of Nursing with current state licensure. Bachelor's degree (BSN) from four-year college or university preferred.
Minimum 4-5 years clinical experience in related field strongly recommended and preferred.
Required to maintain CME's as applicable for licensure.
CPR certification required.
Knowledge of federal, state and local laws and regulations including OSHA and HIPAA requirements.
Knowledge of computerized information systems (EMR) used in clinical management settings.
Perform duties in accordance with the customary rules of ethics and conduct of the applicable State Board of Nursing and other such regulating bodies.
Able to meet the needs of the patient by processing requests and referrals, providing efficient follow up with the patient, pharmacy or other entity, as needed.
Certified Diabetes Educator status is desirable.
Strong commitment to Customer Service and Excellence in healthcare.
Ability to communicate effectively both orally and in writing.
Worksite-based. Requires working in a clinical setting. The noise level is moderate and there is a potential for exposure to infectious diseases and blood-borne pathogens. Also requires: travel to other Worksites/Centers, the ability to stand and sit for hours at a time (with some bending and stooping), ability to use manual dexterity in relation to clinical requirements and ability to lift 50 lbs.
