General Position Description:

The Registered Nurse, Condition Management is responsible for improving the patient's overall chronic disease management and well-being. Acts as an advocate for patients and collaborates with physicians, advanced practitioners, clinical staff and community resources utilizing a proactive, multidisciplinary team based approach directed toward prevention, education and health promotion.

Principal Duties and Responsibilities:

Patient Care (75%):

  • Supports a proactive, multidisciplinary team approach directed toward prevention, education and health promotion.

  • Establishes supportive relationships with the patient and acts as a resource.

  • Assesses patient's educational/clinical needs, prioritizing and individualizing program based on elicited information.

  • Provides disease specific self-care education.

  • Collaborates with Physician or other Licensed Independent Practitioner and other Chronic Condition Management nurses to arrange for further medical treatment when needed.

  • Consults with other members of the healthcare team to solve problems and seek advice. Designs interventions to improve outcomes.

  • Assesses, screens, triages and performs treatments ordered for patients.

  • Identifies emergency situations and provides care within legal scope of practice.

  • Participates in outreach/follow-up phone calls and follow-up patient visits.

  • Assists in the development of criteria to measure outcomes, assists in the analysis of data for quality improvement, interprets study results, identifies limitations and barriers and recommends alternatives.

  • Develops and presents educational services for patients or departments as appropriate.

  • Delivers one on one and/or group coaching sessions in the areas of diabetes, asthma, weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general health.

  • May coordinate and conduct wellness programs in areas such as diabetes, nutrition, stress management, weight management, smoking cessation, exercise and others.

  • Works closely with third party health and wellness vendors.

  • Apply the use of logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions and approaches to problems.

  • Prioritizes and multitasks to optimize patient experience.

  • Ensures the well-being of patients and provides a positive, supportive environment for patients and other staff.

  • Completes patient referrals daily including referrals to client's other health and wellness programs.

  • Collaborates with the client's other in-house and third party health and wellness resources to provide an integrated experience for members.

  • Proper, concise and complete EMR documentation and paper based documentation for all assessments and procedures.

  • Comply with and follow policies and procedures as outlined by MedExpress.

  • Maintains HIPAA at all times.

  • Other duties as assigned such as new models of care and new types of procedures, etc.

  • Provides clear direction to achieve goals creating an environment that fosters team commitment and employee engagement.

  • Maintains the perseverance to drive and sustain the changes that occur at MedExpress while being resilient and flexible and inspiring and motivating the team.

  • Constantly communicates to the team in conjunction with the Lead Registered Nurse, Condition Management; is open to opinions and feedback from team members and follows through on commitments.

  • Creates a work environment in which people are able to perform to the best of their abilities.

  • May participate in obtaining and maintaining patient centered medical home accreditation to include, but not limited to, developing and implementing a quality improvement program, establishing and coordinating exchange of relevant patient information within the guidelines of HIPPA with local healthcare organizations and obtaining and analyzing performance data.

  • Participates in quality improvement projects to include the following topics: patient/primary care provider relationship, accessibility to care, comprehensiveness of care, continuity and/or coordination of care and clinical study.

  • Works with Center Managers and Lead Condition Management Registered Nurse to deliver team based approach to chronic disease management across the health care spectrum.

Learning and Ongoing Education (15%):

  • Maintain skills and competency for all chronic condition management.

  • Maintains technical skills and applies new knowledge to position according to MedExpress policy.

  • Maintains knowledge of Medical Terminology and standards of nursing care.

  • Maintains knowledge of common safety hazards and precautions to establish a safe work environment.

  • Reads and keep up-to-date on all ME Communications.

  • Completes all assigned training.

Center Readiness and Maintenance (10%):

  • Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA and Department of Health).

  • Maintains and complies with all MedExpress, Optum and Urgent Care Accreditation site assessment requirements.

Working Relationships:

  • Worksite clinical team members including providers and non-providers.

  • Worksite Administrative clinical partners.

  • External care services within vicinity of worksite.

Position Requirements

Qualifications:

  • Graduate of an accredited School of Nursing with current state licensure. Bachelor's degree (BSN) from four-year college or university preferred.

  • Minimum 4-5 years clinical experience in related field strongly recommended and preferred.

  • Required to maintain CME's as applicable for licensure.

  • CPR certification required.

  • Knowledge of federal, state and local laws and regulations including OSHA and HIPAA requirements.

  • Knowledge of computerized information systems (EMR) used in clinical management settings.

  • Perform duties in accordance with the customary rules of ethics and conduct of the applicable State Board of Nursing and other such regulating bodies.

  • Able to meet the needs of the patient by processing requests and referrals, providing efficient follow up with the patient, pharmacy or other entity, as needed.

  • Certified Diabetes Educator status is desirable.

  • Strong commitment to Customer Service and Excellence in healthcare.

  • Ability to communicate effectively both orally and in writing.

Working Conditions:

Worksite-based. Requires working in a clinical setting. The noise level is moderate and there is a potential for exposure to infectious diseases and blood-borne pathogens. Also requires: travel to other Worksites/Centers, the ability to stand and sit for hours at a time (with some bending and stooping), ability to use manual dexterity in relation to clinical requirements and ability to lift 50 lbs.

The 55,000 square foot facility is situated on approximately 10 acres in the southeast corner of Alabama, less than 20 miles from both Florida and Georgia. In addition to the living units, the facility houses our full service cafeteria and New Day Academy; a state-supported year-round K-12th grade school with vocational opportunities. We provide numerous recreational activities including an in-ground swimming pool, game room, playground, basketball courts, and gymnasium. The multi-disciplinary staff at Laurel Oaks collaborates to ensure that services provided are clinically and medically appropriate and highly coordinated among all treatment disciplines. Laurel Oaks specializes in the treatment of youth who have multiple challenges and special needs. All children served at Laurel Oaks have a primary psychiatric disorder but most also have co-occurring conditions such as intellectual disabilities, learning disabilities, and/or physical/sexual abuse victimization. Laurel Oaks Behavioral Health Center is currently seeking a full time House Supervisor-Registered Nurse. The House Supervisor-Registered Nurse will report directly to the Director of Nursing and will have the following duties: * Ensures adherence to established policy and procedure within the accepted scope of nursing practice.Evaluates the needs of the patient and makes nursing assignments considering staff appropriate knowledge and skill mix for facility. * Utilizes the nursing process in planning, providing, and evaluation each patients care, and documents pertinent information properly. * Utilizes information from the Nursing Assessment to identify patient problems and identifies appropriate nursing interventions for Master Treatment plan * Provides guidance to nursing staff on Nursing practice and facility policy and procedures when needed, supervises all nursing staff to ensure compliance with all regulatory standards during assigned shift. * Serves as role model for patients and staff members. * Must have credentials to direct physical management of the patient, and has current CPR certification. * Assists with physical exams. * Observes patient behavior for changes in order to intervene therapeutically, guide non-professional nursing staff, communicate changes to appropriate staff, and record changes. * Maintains confidentiality and has ultimate responsibility for security of records on the nursing unit. * Consistently demonstrates competence in day to day scheduling of duties by completing tasks on time with little assistance from others. * Always exhibits a positive attitude when dealing with others in day-to-day work situations. * Maintains and adheres to facility employee attendance guidelines. * Adapts patient care in accordance with individual patient needs bases on age and level of development. * Safely prepares, administers, and monitors results of medications as evidenced by documentation on the Medication Administration Record, documentation on the medication observation record and by accountability of the PYXIS control records. * Responds to Code R's and therapeutically intervenes with the patient in crisis as evidenced by completion of seclusion/restraint forms indicating the necessity of any seclusion or restraint, observation of the patient and documentation of patient's condition, criteria for discontinuation of seclusion/restraint and physician notification * Ensures safe and appropriate after-hours transportation as needed. * The Nurse House Supervisor is responsible for understanding and demonstrating the theory and skills required to reduce Seclusion and Restraint and other restrictive measures as well as physical and emotional risk factors. The House Supervisor-Registered Nurse will have the following qualifications: * Graduate from an accredited School of Nursing. * Bachelor's Degree in nursing (preferred.)A diploma graduate and/or an Associate's degree in Nursing required * Current status in good standing as a Registered Nurse in the State of Alabama. * Two (2) years psychiatric nursing preferred.
