Position Summary

The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization's vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance

Education

Required:

Associates Degree

Bachelor's Degree (New Hires as of 4/1/2019)

Experience

Required:
3 Years Healthcare experience

Licensure

Currently licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation.

BLS

ACLS (within 6 months of hire)

PALS (within 18 months of hire)

TNCC (within 18 months of hire)

BASIC FUNCTION: This position is responsible for conducting medical management activities including condition management, care coordination, and performing assessments to create a care plan. JOB REQUIREMENTS: * Registered Nurse (RN) with 2 years of direct clinical care as an RN to the patient/consumer in a clinical setting. * Registered Nurse (RN) with current, valid, unrestricted license in The State of Texas (or reciprocity). For compact licensee changing permanent residence to state of operations, you must obtain active, unrestricted RN licensure in the state of operations within 90 days of hire. * Plus 3 years wellness or managed care experience as an RN presenting clinical issues with members/physicians. * Knowledge of the health and wellness marketplace and employer trends. * Verbal and written communication skills including composing and conducting presentations, facilitating client specific programs, and interfacing with internal staff/management and external vendors and community resources. * PC proficiency to include Word, Excel, and PowerPoint, database experience and Web based applications. * Organizational skills for maintaining a library of presentation of interventions and outcomes. * Ability to travel occasionally as needed including overnight stays. * Ability and willingness to provide coverage after hours on a call rotation. * Incumbents with nursing licenses in positions/departments requiring multi-state licenses are required to obtain and maintain additional current, valid, and unrestricted applicable nursing licenses in other states as determined by management. Multi-state license fees will be provided by HCSC. Incumbents with other clinical licenses are not required to obtain multi-state licenses. PREFERRED JOB REQUIREMENTS: * 3 years of clinical experience as an Oncology RN. * Oncology Certified Nurse (OCN) or other relevant Oncology nursing certification is a plus. * Patient Education, Condition Management, Disease Management and Case Managment experience. * Recent experience either telephonic or in person clinical support working with patients with chronic illnesses, including Diabetes, CAD, hypertension, CHF, COPD. * Bilingual in English and Spanish preferred. * Experience as an RN managing complex or catastrophic cases. * Case Management Certification or CDE. * BSN * Experience with Motivational Interviewing or other behavior change communication techniques. * Experience in working with a dedicated unit or custom account/client management. * CA HCSC is committed to diversity in the workplace and to providing equal opportunity and affirmative action to employees and applicants.
