The Position: We are looking for a full-time Registered Nurse Case Manager(RN) to provide case management for members who have complex needs to ensure cost effective quality of care. The client offers the following compensation package:

• $72,000- $92,000 Salary Based on Experience

Responsibilities:

• Monday- Friday Schedule, Traveling to see Patients in Their Homes in and Around the Lynn Area
• 8:00am- 4:00pm
• No Nights
• No Weekends
• Not a Direct Bedside Care Position
• Provide Case Management to Patient's in Their Homes
• Setting Up Services Based on Physician's Orders
• Develop a Plan of Care Based on Physician's Orders
• Update Plan of Care on a Regular Basis
• Collaborate with Physicians
• Provide Information to Both the Patient and Their Families

Requirements:

• MA RN Licensed
• 2+ Years of Experience as a Nurse
• 2+ Years of Home Care Experience Preferred, But Not Required
• CPR Certified
• Works With Interdisciplinary Team
• Build Strong Relationships With Participants
• Must Have Great Communication Skills
