To determine the medical necessity of all admissions and the need for continued stay. To identify inappropriate or untimely services, focusing on opportunities to reduce inappropriate patterns of utilization.
Discharge planning assessment of all ages to begin in the pre-hospital phase, minimal upon admission to facility. To coordinate the delivery of services to meet patient's discharge needs. To be capable and comfortable offering alternatives in patient care (Home Health, SNF, ALF, ect.). To facilitate timely and appropriate discharge from the hospital.
To identify opportunities for improvement in patient care. To provide documentation of all functions as required by regulatory agencies.
2 year Associates Degree
FL RN Licensure, or Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provision)
3-5 years experience of combined experience with clinical nursing and case management experience
Clinical acute care nursing experience from a hospital required
