Essential Job Functions

The Registered Nurse Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

  • Completes comprehensive assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es) on an ongoing basis.
  • Regularly re-evaluates patient nursing needs and updates nursing care plans on an ongoing basis.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan which establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Processes orders and notifies physician of patient needs and changes in condition.
  • Completes certification/recertification orders and discharge summaries.
  • Supervises and teaches other paraprofessionals, assists in training and orientation of new hire RN Case Managers, serves as a peer mentor.
  • Additional Duties
    • Participates in on-call duties as defined by the on-call policy.
    • Ensures that arrangements for equipment and other necessary items and services are available.
    • Instructs, supervises and evaluates PCA and/or home health aide care provided as required by regulation
    • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
    • Fulfills the obligation of requested and/or accepted case assignments.
    • Actively participates in quality assessment performance improvement teams and activities.

(Typical caseload is 40-45 individuals)</li> <li>Assisting the person/client to access services, develop service agreements and documentation through case notes</li> <li>Informing the person or legal guardian of service options that are a fit for their needs</li> <li>Meeting one-on-one with each client at least twice a year to evaluate progress and to adjust the client’s needs as needed.</li> <li>Help Identification of potential providers that will guide each client towards those goals</li> <li>Evaluation and monitoring of the services identified in the plan</li> <li>Making sure to follow the case management rule 185</li> <li>Complying with MN Health Care Programs in relation to the delivery of waiver services (such as CADI, BI, EW, DD) including having MnChoices assessments completed with individual plans in place and using state systems as required.</li> </ul><p><strong>Requirements</strong></p><p><strong>Credentials/Qualifications:</strong></p> <ul> <li>1+ of experience in the delivery of social services to adults with mental and physical disabilities</li> <li>BA/BS in social work, nursing, psychology, sociology or a closely related field</li> <li>Experience working with adults with mental health diagnoses, homelessness, or complicated medical conditions</li> <li>Good written and oral communication skills</li> <li>Excellent at working independently and in crisis situations</li> <li>Comfortable with high-volume paperwork</li> <li>Attention to detail and process oriented</li> <li>Able to pass a DHS (Department of Health Services) background check, valid driver’s license and clean driving record</li> <li>Reliable transportation as you will be going from client to client</li> <li>Experience in ILS is preferred</li> </ul><p><strong> <strong>Work Environment: </strong></strong></p> <p>The employee in this position typically works the majority of the time in the community and office setting. Candidates will be expected to work generally Monday – Friday, daytime hours, with some flexibility based on program needs.
