Case Manager Registered Nurse (RN)

Position Summary: Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Quality and Risk Management issues will also be monitored for and reported to the QRM department as a part of the CQI process.

  • Registered Nurse, Colorado Licensure. BLS certification.

  • Graduate of an accredited school of nursing; BSN preferred.

  • 3-5 years recent clinical or case management experience in area of specialization.

  • Requires excellent human relations and oral/written communication skills. Must have the ability to asses information, apply critical thinking processes to determine course of action, define expected outcomes, and evaluate effectiveness of plan through continuous assessment of the patient's progress. Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews. Must also have strong analytical and organizational skills.
    Minimal supervision: Individual proceeds on his/her own initiative in compliance with policies, procedures and practices prescribed by immediate supervisor.
    Orientation Period: 90 day probationary period. 2 3 weeks training/orientation contingent upon experience. Extensive orientation may be required and will be determined by the Department Director.

Our client, a medical facility in Denver, is looking for RN Case Managers for a temporary contract assignment (possibility to convert to fill time). <br /> <br /> Job Responsibilities <ul> <li>Coordinate with doctors, technicians, and other clinical personnel in the development and implementation of patient care plans.</li> <li>Speak with outside providers to arrange for the emergency care and (if necessary) admission of patients being brought in by emergency medical service teams or being transferred from other facilities.</li> <li>Work side-by-side with other members of our clinical teams in delivering direct care to patients under the direction of our physicians.</li> <li>Provide end-to-end care to patients, including initial assessments and triage, coordinating laboratory, pathology and imaging services, following patients admitted to facility to their respective care units, and then overseeing care up to the point of discharge.</li> <li>Coordinate the daily care of patients admitted to our facility, ensuring that medications are ordered and administered when needed, therapy schedules are followed, and that patient's dietary needs are met.</li> <li>Prepare patients scheduled for surgery in accordance to the orders handed down from our surgical team, and then maintain a sterile environment in their rooms in preparation for them to return for recovery.</li> <li>Enter all information from notes and documentation made throughout the day into a patient's electronic medical record.<br />  </li> </ul> Job Skills & Qualifications<br /> <br /> Required <ul> <li>Bachelor's or Associate's degree in nursing</li> <li>Current RN licensed to the current standards of the state</li> <li>CPR certification</li> <li>Experience providing direct services to individuals with complex medical conditions, mental health, and/or substance abuse</li> <li>High comfort level of working over the phone</li> <li>Demonstrated knowledge of infection control and cross contamination prevention guidelines<br />  </li> </ul> Preferred <ul> <li>5+ years of case management experience working with diverse patient populations</li> <li>CCM Certified</li> <li>Will be working in Guiding Care software</li> </ul> Opulent Solutions LLC Denver CO
