Residential Home Health and Hospice is seeking Registered Nurses who share the same passion and mission for providing excellent care to patients and caregivers to join our hospice team as a Hospice Registered Nurse Case Manager.

Comprehensive benefit program,
Assess the physical, psychological, social, and spiritual needs of the patient and caregivers and formulate a comprehensive Interdisciplinary plan of care.
Coordinate with physicians, caregivers, volunteers, ancillary therapies, and home health aides to support the care and needs of the patient.
Educate on prescribed medications and supplies and ensure optimal quantities.
Oversee and document home health aide and licensed practical nurse care plans.
Attend interdisciplinary team meetings to evaluate the plan of care and revise as needed.
Provide support at time of death and period of bereavement as needed.
Document patient and family care plans, actions, goals, and concerns in Homecare Homebase (HCHB), Residential's EMR, through a company-provided tablet.
Must hold, in good standing, a license as a Registered Nurse, issued by the state in which you work.
A minimum of one (1) years of experience working as a Registered Nurse including some medical/surgical experience and/or an acute care or home care setting is preferred.
Must understand the hospice philosophy and team approach to hospice care.
Must be able to transfer patients as needed according to proper techniques of body mechanics to ensure patient and staff safety.
Must have strong communication and organizational skills and high-standard of ethics and compliance.
Ability to document within an electronic medical record.
Associate Degree from an accredited School of Nursing or higher preferred.
