Registered Nurse - Case Manager (Hiring Immediately)

Unit Description: The Case Manager at Elizabethtown Community Hosptial (ECH) coordinates the care and services of selected patient populations across the continuum of health; promotes effective utilization and monitoring of health care resources; and assumes a critical role with the interdisciplinary team to achieve optimal patient-centered, clinical, and resource outcomes.

They will e nsure efficient and effective discharge planning and referral functions to maximize throughput and transition of care. They proactively identify and intervene to address barriers to treatment, health outcomes, and discharge planning, including biopsychosocial and other high-risk factors. They are an essential link between the patient, the healthcare team, the payer, and the community and work diligently to support their patients’ appropriate transitions of care.
