We’re working on behalf of a 231-bed hospital that provides comprehensive medical care that treats each patient holistically. Their team has worked diligently to ensure that the residents of the South Bay do not need to travel outside of the region for any medical service or specialty care.We seek a California licensed Registered Nurse with at least two (2) years of care coordination experience in an acute care hospital setting for the role of CARE MANAGER RN - FULL-TIME/DAYS. The person in this role will join a collaborative practice that includes caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. She/he will perform the primary functions of assessment, planning, facilitation and advocacy. She/he will facilitate communication and care coordination along a continuum through effective transitional care management. She/he will recognize the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).
Candidates must have a Bachelors degree in Nursing and be licensed as a Registered Nurse in the state of California. Must have two (2) years of acute care hospital experience in care coordination.The role offers highly competitive compensation (salary to $149,700 based on experience) and excellent benefits.
