Position Summary

As a member of the Inter-Disciplinary Team, the RNCM will work as a hospice field nurse to provide both routine and emergency assessments as well as evaluative services to meet the needs of the patient and their family. You will help maintain their maximum level of comfort and coping, flex your schedule to meet agency-staffing needs, and provide after-hours emergency support to the patient and family.

Principal Accountabilities

  • Coordinate clinical, psycho-social and spiritual services as indicated by Plan of Care through case management

  • Provide physical and emotional intensive care to support the patient and family in periods of crisis

  • Collaborate with the Inter-Disciplinary Team to coordinate hospice care for the patient and family to ensure appropriateness, continuity, and quality of care

  • Explain hospice services and Medicare benefits to patients and families

  • Oversee the care given by Licensed Practical Nurses/Licensed Vocational Nurses and Hospices Aides


  • Strong leadership and organizational skills required

  • Effective communication and compassionate interpersonal skills

  • Knowledge of Microsoft Office programs

  • A valid driver's license and auto liability insurance

Education and Certifications

Registered Nursing License in state of employment. Minimum of one year nursing practice or equivalent experience. Hospice, Oncology, Home Health and/or Long-Term Care experience preferred.

State-specific certifications

  • Louisiana: Two years full time experience as a Registered Nurse or two years clinical in hospice

  • Mississippi: 1 year full time experience as a Registered Nurse or 3 years full time continuous employment as a Licensed Practical Nurse immediately preceding Registered Nurse licensure

This position is responsible for bringing services, agencies, resources and other people together to assist a client in achieving established goals. This position will cover a large area in the East Central, IL with primary ofice will be out of Erringham, IL. Counties that will be covered are: Clark, Effingham, Clay, Rayette, Coles, Jasper, Crawford, Lawrence, Cumberland, Marion, Douglas, Mourltrie, Edgar and Richland Essential Duties and Responsibilities * Provide direct services and coordinate other services, resources and people to assist clients in achieving their established service plan goals * Provide specialized case management services to clients and their families with mental health and other needs. * Screen and assess clients for their appropriateness for agency services * Adheres to the service hour protocol established by the agency * Support engagement in drug treatment, community, etc. * Counsel and aid family members to assist them in understanding, dealing with, and supporting the client or patient * Facilitate individual and/or group orientation or educational sessions * Facilitate client drug test * Represent clients and agency at staffing and court hearings, probation, parole and provider networks * Provide referrals for identified needs of the client and crisis intervention when needed * Document the results of assessments, service plans, reports, progress notes, discharge summaries and other client-related activities including accurate reporting of services * Attend in-service trainings, conferences and education sessions Qualifications * Bachelors degree preferred in human services, social work, psychology or other related fields * Two or more years experience in human service activities; or any equivalent combination of training and experience * Knowledge of human behavior for the assessment and signs and symptoms of substance abuse disorders. Specific knowledge necessary for working with special populations. * Knowledge of the signs and symptoms of substance abuse including pharmacological factors * Knowledge of the ASAM placement criteria and the DSM-IV as it relates to substance-related disorders or able to acquire the knowledge * Possess clinical skills to effectively manage clients * Ability to obtain CARS or CADC certification through the State Certification Board (IAODAPCA) within the first two years in the field (if required by program). * Knowledge of the Code of Ethics for substance abuse professionals. * Knowledge of the federal drug and alcohol confidentiality law (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act * Ability to work with a diverse population * Knowledge of the criminal justice system, Knowledge of computers, including proficiency in Microsoft Office applications and the Internet * Must be available to attend meetings and meet the needs of the program and/or clients throughout the city, counties or wherever needed (a reliable car, insurance and valid drivers license may be required for the job) Qualified candiates must successfully pass and maintain IDOC background clearance: daily travel required; may require going into correctional facilites as needed.
