The Staff Nurse will be responsible for:

Providing care to adolescent and adult patients with congenital and acquired heart disease within the Boston Adult Congenital Heart (BACH) Program who are cared for at Boston Children's and Brigham and Women's Hospital
Phone triage of routine and urgent concerns in collaboration with APP and MD colleagues
Pre-clinic preparation - route orders for diagnostic testing for MD /APP cosign
Prescription refills - via phone or route via e-prescribe to MD /APP prescriber for cosign
Support for prior authorizations, referrals, letters of medical necessity
Patient and family education and anticipatory guidance
Follow-up phone calls regarding test results and follow-up recommendations
Working 40 hours per week: Monday through Friday, either five 8 hour shifts or four 10 hour shifts

To qualify, you must have

Bachelor of Science in Nursing
Current MA RN license
BLS certification
2 years of adult cardiac or pediatric cardiac RN experience (3-5 years preferred, acute care inpatient preferred)
EMR competency
Strong organizational, and critical decision making skills
Strong written and verbal communication skills
Flexible and collaborative approach to patient care
Ability to travel to satellite locations if operational need

VIEW JOBS GENERAL SUMMARY/ OVERVIEW STATEMENT: The physician assistant (PA) is a licensed provider who has acquired a broad range of knowledge and skills through the successful completion of a physician assistant program. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. The physician assistant works under the supervision of an attending physician and reports directly to the department administrator or chief physician assistant administratively. The PA will perform in accordance with all state and federal regulations and institutional policies including but not limited to the BWH PA Scope of Practice, Prescriptive Guidelines, approved clinical privileges, and BWH PA policy. identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation * Demonstrates understanding of procedures, policies, and documentation required to ensure compliance with hospital standards of care * Additional duties and responsibilities as required by the department/division For all positions that include direct patient care, indicate with an "X" the age(s) of all patient populations served No Direct Patient Care All age groups Adolescence (13 to 17 years) X Neonates (birth to 1 month) Young Adult (18 to 25 years) X Infant (1 month to1year) Adult (26 to 54 years) X Early Childhood (12 months to 5 years) Senior Adult (55 to 64 years) X Late Childhood (6 to 12 years) Geriatric (65 years and up) X QUALIFICATIONS: * The appropriate candidate will have successfully completed a graduate level physician assistant program (Master's degree) accredited by the Accreditation and Review Committee for Physician Assistants (ARC-PA) or such other accrediting agency as may qualify a graduate to sit for the NCCPA certifying examination * BS/MS joint programs and those candidates with a BS and PA certificate are eligible, but ONLY for those who graduated BEFORE 1995 * Experience in a surgical specialty, cardiology and/or adult congenital cardiac surgery setting preferred. * Certification by the National Commission on Certification of Physician Assistants (NCCPA) must be obtained prior to first day of employment * Must hold a current valid license or be eligible for licensure as a physician assistant in the Commonwealth of Massachusetts * Must have or eligible for, and maintain, a current DEA registration and Massachusetts Controlled Substance Registration * Must complete 100 hours of CME according to national and state regulations during each two year period of employment * Must complete the BWH Professionalism Program within three months of date of hire * Must complete the BWH Procedural Sedation Training if deemed necessary SKILLS/ ABILITIES/ COMPETENCIES REQUIRED: * Must have the ability to work in a high stress, dynamic, busy clinical setting with changing organizational climate * Must have the ability to be self-directive and to demonstrate initiative in addressing the responsibilities of the position * Must have the ability to utilize appropriate problem-solving and conflict resolution skills * Must be energetic and quality driven and have a proven ability to be a productive member of a team dedicated to providing optimal patient care * Must be proficient with BWH computer systems as needed to independently perform daily tasks (EMR, Epic preferred) * Must maintain the highest degree of professional judgment under the supervision and guidance of a licensed physician WORKING CONDITIONS: Work settings may include ambulatory clinics and treatment rooms, inpatient areas, intensive care units, operating rooms, and/or procedural areas. There may be loud noises, noxious odors, and temperature extremes. A large portion of the workday may be spent either standing or walking and the position may require heavy lifting of equipment and patients. The PA may be exposed to radiation, potentially infectious bodily fluids, and sharp instruments such as needles and scalpels. Knowledge of Universal Precautions and proper use of surgical sharp instruments may be required. The consequences of contamination from bodily fluids or from being stuck with a sharp instrument include the possibility of physical harm to the employee or others. BWH is a fast paced, dynamic environment with a very large and complex patient volume; exposure to stress in the workplace is expected. SUPERVISORY RESPONSIBILITY: The PA I does not have supervisory responsibilities. FISCAL RESPONSIBILITY: The PA I does not have fiscal responsibilities. ADDENDUM: The Physician Assistant I of Adult Congenital Cardiac Surgery is a full-time position with a consistent schedule of four ten-hour days and does not require working on weekends, holidays or nights. The PA of ACCS will have dual privileges at both Boston Children's Hospital and Brigham and Women's Hospital, work closely with cardiac surgeons from BCH and work exclusively at BWH in the operating room, outpatient clinic, cardiac surgery ICU and step-down units with all ACCS patients. The Division of Cardiac Surgery believes in the "team" approach to patient care. In collaboration with Boston Children's Hospital Cardiac Surgeons, the PA I of ACCS will be involved in coordination of treatment and assistance in management of preoperative and postoperative adult congenital cardiac surgery patients. The PA is required to have a clear knowledge of adult congenital cardiac surgery and work autonomously while being proactive with patient care. PRINCIPAL DUTIES AND RESPONSIBILITIES: The Physician Assistant I of Adult Congenital Cardiac Surgery will: * Attend and contribute to all pre-operative consultation and post-operative clinic visits of the adult congenital cardiac surgery patient in addition to assisting the Boston Children's Hospital cardiac surgeon attendings in the BWH operating room, cardiac surgery ICU, step-down units and outpatient clinic. * Perform preoperative evaluation of the ACCS patients, including but not limited to the patient's medical and surgical history, physical exam findings and medication review and reconciliation while documenting all information in the patient's electronic medical record. * Act as the liaison for the BCH attending cardiac surgeons and ACCS patients, beginning with the patient's initial pre-operative consultation, throughout their hospital course and post-operatively. * Provide direct care, educate, counsel and support all ACCS patients and their families in the ambulatory and inpatient settings while being readily available to answer all questions, respond to phone calls and provide resources to augment learning. * Obtain, review and interpret all pre-operative and post-operative cardiac imaging results, cardiac catheterization results and labs and communicate findings to the attending BCH cardiac surgeons. * Schedule all required pre-operative imaging and obtain results to communicate to the BCH cardiac surgery attendings while working closely with surgical schedulers to assure specific and accurate surgical procedures * Communicate and work collaboratively with the BWH inpatient and outpatient cardiac surgery PA team, ICU attendings and residents/fellows regarding patient management, concerns and challenges to discharge of the ACCS patient and discusses all information with the BCH cardiac surgery attendings in a timely manner. * Communicate and work collaboratively with VNAs, PCPs, cardiologists and pharmacists of the ACCS patient, both pre-operatively and post-operatively * Perform post-operative medicine reconciliation and pain management. * Develop appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings * Order medications and write prescriptions according to organizational and regulatory policies and procedures * Organize and teach weekly ACCS education sessions for all nurses, PAs, perfusionists, ICU staff, residents and fellows on the BWH cardiac surgery service in reference to upcoming scheduled adult congenital surgical cases * Attend and participate in weekly Boston Adult Congenital Heart meetings to review and discuss potential ACCS patients * Consistently provide high quality and timely documentation * Perform procedures as appropriate for the ACCS patient * Practice cost effective medicine in an efficient manner, maximizing available resources * Discuss patient data with BCH cardiac surgeons, ICU attendings, Boston Adult Congenital Heart cardiologists, and cardiac surgery team members as appropriate in a multidisciplinary setting * Demonstrate expert clinical judgment skills to function in an independent role * Additional duties and responsibilities as required by the department/division Non-Clinical Duties and Responsibilities: * Administrative responsibilities, postop phone calls, communication to referring providers, discharge calls and documentation, interactions with VNA, prescriptions and refills. * Demonstrates professional collaborative and consultative relationships with other providers * Follows all designated prescriptive practices and monitoring requirements with the designated supervising physician and administrative report * Participates in departmental and organizational committees and programs as appropriate and attends all required meetings, in-services, and professional trainings * Maintains superior interpersonal and communication skills as a member of the healthcare team in order to collaborate effectively with patients, families, staff, and community health workers * Remains actively involved in continuing education with a commitment to self and departmental growth * Adheres to all established safety policies, procedures, and precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation * Demonstrates understanding of procedures, policies, and documentation required to ensure compliance with hospital standards of care * Additional duties and responsibilities as required by the department/division

GENERAL SUMMARY/ OVERVIEW STATEMENT: The Mechanical Circulatory Support (MCS) Staff Nurse will assist in the medical and surgical management of patients with end-stage heart disease that are being evaluated for, undergoing, or have undergone implantation of mechanical circulatory support. Primary responsibilities will include: 1) supporting and extending the clinical activities of MCS physicians and nurse practitioners; 2) collecting and entering quality and performance measure data into MCS databases, as required by the hospital, and by local, regional, and national agencies; 3) managing MCS equipment issues; and 4) assisting in MCS research activities. PRINCIPAL DUTIES AND RESPONSIBILITIES: Support and Extension of Clinical Activities: * Coordinates evaluation of advanced heart failure patients being considered for MCS, including left and/or right ventricular assist device (VAD). Responsibilities will include organization of cardiac and non-cardiac testing; collecting and reporting results; entering clinical information into electronic databases; and sharing information with internal and external providers as needed. * Collaborates with unit staff, MCS nurse practitioners, MCS unit physician assistants (PA), and physicians to facilitate a comprehensive plan of care and coordinate resources. * Coordinates with a multitude of multidisciplinary MCS patient teams that can include but are not limited to physicians, nurses, social workers, pharmacists, physical and occupational therapists, and care coordinators to ensure that a patient is an appropriate MCS candidate. * Maintains continuous communication with and training of community agencies such as, but not limited to, local EMS or community hospitals regarding ongoing care and support. * Maintains all MCS-related information that supports management of the MCS program. * Communicates anticipated patient needs to appropriate physicians, PAs, nurses, and admitting departments. Maintains telephone contact with patients to assess clinical status, monitor response to and possible complications of MCS therapy, adjust medications in consultation with clinical providers and organize outpatient or inpatient follow-up as needed. * Assists in urgent or emergent MCS patient triage, including emergency room visits and transfer from outside institutions and facilitation of scheduled clinical visits, and documents care provided. * Collaborates with inpatient nursing care specific to patient travel to tests, cardiac catheterization laboratory, or operating room as well as discharge education and planning * Provides education to MCS patients and family members as well as inpatient staff nurses and residents regarding important aspects of care, including but not limited to device function and maintenance, activity level, diet, medication adherence and healthcare follow-up. Helps to maintain current supply of patient education materials. * Works collaboratively with biomedical engineering to maintain up-to-date and safe equipment for MCS patients, including pumps, power modules, batteries, cables and other peripherals as well as ensuring that adequate supply is on hand for planned and emergency needs. * Sets up consults with specialty providers, follows up consult recommendations and communicates results to MCS patient and other providers. * Assists with communication and correspondence to/from patients, employers, pharmacies and insurance providers with regards to medication prescriptions, cardiac rehabilitation, disability, family medical leave, and payment coverage. May also communicate and correspond with other individuals/companies (e.g., utility company, telephone company) as dictated by patient needs. * Collaborates with the creation and maintenance of clinical practice guidelines and policy and procedures as relevant to the care of the MCS patient and family. * Maintains BWH MCS patient database. * Provides support to other areas within the Center for Advanced Heart Disease such as but not limited to HF office, ACTIVE center, HF inpatient/outpatient coordination as well as heart transplant and MCS evaluations. * Other duties as assigned. Regulatory: * Collaborates with MCS NPs, PAs, physicians and nursing leadership, as well as hospital quality and compliance personnel to ensure MCS regulatory compliance as defined by Joint Commission standards. * Assists in maintaining a state of continuous regulatory readiness. * Collaborates with the creation and maintenance of clinical practice guidelines and policy and procedures as relevant to the care of the MCS patient. * Participates in quality assurance/process improvement, infection control and morbidity and mortality initiatives. * Maintains patient database. * Attends device specific education classes, VAD QAPI meetings, and assist in arranging education for the multi-disciplinary VAD team members. QUALIFICATIONS: * Bachelor's degree in nursing required * Minimum 3 years of experience (in the last 5 years) of direct advanced heart failure and/or MCS-focused patient care experience required * Experience with advanced heart failure symptom management telephone triage in the ambulatory patient care setting required * Current, within the last 2 years, ambulatory cardiac patient care experience required * Current, within the last 3 years, cardiology-focused patient care experience strongly preferred * Current, within the last 3 years, experience with regulatory survey preparation strongly preferred * Basic knowledge of hospital, industry, and agency standards for procedures used to collect, compile and present clinical and research data.
