The Registered Nurse, as a member of the interdisciplinary team, is responsible for the provision of skilled nursing assessment and provision of patient care in order to maximize the palliation of symptoms. In addition, the registered nurse is responsible for identifying patient or family needs and providing supportive care and appropriate services in accordance with the attending physician's orders, the interdisciplinary team plan of care, and the policies and procedures of the agency.


1.Be eighteen (18) years of age or older

2.Minimum associate degree in nursing from an accredited school

3.Possession of a current license to practice in the State of New Mexico as a registered nurse.

4.Demonstrated skill in nursing practices and clinical judgments

5.Ability to cope with family emotional stress and tolerance of individual lifestyles

6.Understanding of the hospice and home health care philosophy and the ability to assess and manage symptoms

7.Must have a valid NM Driver's license

8.Must have reliable transportation available during work hours. Vehicle must be properly registered and insured in accordance with state law.

9.Must possess current CPR and First Aid Certification


1.Provide skilled nursing assessment and implement patient care as directed by the attending physician.

2.Obtain data on physical, psychological, social and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team.

3.Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.

4.Effectively utilize resources to assist in meeting the patient's needs.

5.Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician.

6.Understanding of Home Healthcare philosophy and ability to assess and manage




7.Perform initial assessment of individuals referred for care, determine eligibility under

Ambercare criteria and perform admission of the patient.

8.Maintain an accurate clinical record through the timely preparation of clinical notes.

9.Maintain regular communication with all members of the interdisciplinary team, alerting physician and nursing supervisor to any changes in the patient's condition or needs as well as potential problems or issues.

  1. Provide effective teaching to family, caregivers and interdisciplinary team members to ensure continuity of care for the patient.

  2. Develop and maintain familiarity with Federal, State and Ambercare policies and procedures.

  3. Supervise and assess the delivery of care by the Home Health Aide and the Licensed

Practical Nurse as needed.

  1. Share in providing 24 hour, 7 day per week coverage to patients and families as necessary.

  2. Accept responsibility for professional growth and development through participation in continuing education programs, agency in-services and community events.

  3. Adhere to dress code

  4. Adhere to work schedule and maintain good punctuality and attendance record


1.Satisfactory completion of pre-employment health screening if appllicable, including drug screening.

2.Satisfactory completion of background check in accordance with State of New Mexico

Caregivers Criminal History Screening Program.


Moderate to Heavy--Heavy lifting necessary for patient care. Must be adaptable to a variety of environments and community settings. Driving in all types of weather required.


Varies by agency needs and department assignment; Typically On-Call or PRN hours as assigned by nursing supervisor.

Responsibilities Key responsibilities for this position include: * Provide high-quality, skilled, in-home Infusion therapy (most often prescribed weekly. Anticipated infusion time is 1 hour). * Complete visit specific nursing documentation and submit to EVERSANA Life Sciences. * Provide focused clinical education and support to patients, family and caregivers, as appropriate. * Establish the prescribed visit/infusion timeline in conjunction with the patient and/or caregiver's availability. Communicate with the patient and/or caregiver directly regarding visit scheduling. * Reconcile medications and supplies to ensure availability for upcoming infusions. * Communicate with prescribing physician, EVERSANA Clinical Nurse Coordinator and Pharmacists any and all significant changes (ex. Dose or infusion changes, compliance issues, patient overall condition, issues of therapy tolerance) while strictly adhering to EVERSANA HIPAA regulations * Participate in on-going quality improvement initiatives (ex. On-site visits accompanied by an nurse surveyor, documentation review, patient surveys, and other measurement/analysis tools). Qualifications Requirements for this position: * A current, active and unrestricted RN license in your state * Professional Liability Insurance (greater or equal to $1mil/occurrence and $1mil/aggregate) * Current AHA BLS for Healthcare Providers certification * Completion of background check and drug screen * Completion of EVERSANA provided training. * Contractually required to shield patient's protected health information. * Access to and ability to operate private and secured computer, internet, email, telephone and voicemail * Analytical, critical-thinking and interpersonal communication skills, required. * Recent IV therapy experience, required. * Competence and comfort in responding to emergencies, severe allergic reactions and/or anaphylaxis. * Recent home care experience, preferred. * Critical care nursing experience, preferred EVERSANA™ is the leading independent provider of global services to the life science industry.
