Description:RESPONSIBLE FOR:Assesses the nutritional status of geriatric, adult, adolescent and pediatric patients.Develops and revises an evidenced based nutrition care plan to meet patients nutrition needs.Provides nutritional support to patients receiving enteral or parenteral nutrition
Educates patients and family members to ensure understanding of appropriate nutrition needs to support illness, promote recovery and improve function
Participates in multi-disciplinary rounds, committees and support groups
Acts as preceptor to nutrition interns
