Status: Full Time, 32 per week, 4 days per week, no weekends, or evenings

Shift: Days, 8:00am - 4:30pm

Location: Thome PACE | Jackson

Thome PACE serves Jackson, Lenawee, and Hillsdale counties providing participants a wide range of medical and supportive services in our Day Clinic located in Jackson, Michigan or in the comfort of their own homes. The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve. Thome PACE (Program of All-Inclusive Care for the Elderly) provides a welcomed alternative to nursing home care for participants. The PACE model of care allows seniors aged 55+ to remain independent in their homes and communities while receiving the integrated social, wellness, and medical care services aging adults require.

Overview:

The Registered Dietitian (RD) applies advanced dietitian knowledge and skills to provide personalized nutritional care plans for older adults.

What you'll do:

  • Performs nutritional assessment and reassessments

  • Identifies need and implements nutrition services

  • Provides nutrition education to participants and caregivers

  • Attends meetings and takes part in participant care planning

  • Works collaboratively with other IDT members

  • Documents changes in medical records

  • Assists in monitoring meals

  • Coordinates 24-hour care delivery

What it Takes:

  • Bachelor's degree or master's Degree from an ACEND accredited program and completed the Dietetic Internship.

  • Passed the CDR exam and maintain Michigan Registered Dietitian (RD) status in good standing. Those who are eligible to sit for the CDR exam may be considered.

  • Must be CPR certified or willing to obtain CPR within 90 days if offered employment.

  • Master's Degree plus approved work experience preferred.

  • Certified Diabetes Educator (CDE) status preferred.

  • Minimum of one (1) year working with the frail and elderly.
