A practitioner with a level of knowledge, skills and experience in dietetics practice necessary for the attainment of a Registered Dietitian credential.
Plans and administers assessment, education and other activities related to resident/patient nutrition care and meal service in compliance with the standards established by Aramark Healthcare, regulatory agencies and client. Establishes and maintains effective multi-disciplinary working relationships to provide a unified approach to patient/resident care.
Assists with programs, services or curricula that impact dietetics or promote optimal patient care.
- Conducts and documents timely patient nutrition assessment with nutrition diagnosis; develops and implements an individualized plan for nutrition intervention in accordance with the patient's medical program goals and objectives, nutrition prescription, and self-management training. Monitors and evaluates the effectiveness of, and patient response to, evidence-based medical nutrition therapy interventions.
Assesses educational needs and the presence of barriers to learning. Provides nutrition education or nutrition counseling for individuals and groups. Considers needs of age-specific populations as well as cultural, religious, and ethnic concerns to optimize patient learning.
Evaluates achievement of learning objectives by the patient and/or family. Provides appropriate follow-up in accordance with the patient's treatment goals. Refers patients for outpatient counseling, community, or home health services as appropriate.
Establishes and maintains effective working relationships to provide coordinated, high quality care. Works cooperatively with the medical staff and the food and nutrition services staff to assure conformance to prescribed nutrition care orders. Participates in multi-disciplinary patient/resident care conferences, team meetings or rounds.
Maintains clinical productivity standards and accurate record keeping of daily clinical activities, including information for scheduling and billing. Participates in community related nutrition event opportunities. Assists with Food Service staff development programs.
Assists with performance planning process and competency assessment of FSAs to ensure ongoing improvement in performance. Participates in departmental or patient care performance improvement projects. Assists with data collection and evaluation of indicators to monitor the quality and appropriateness of patient/resident care.
May serve as co-investigator in dietetics research projects. Perform other duties as assigned. Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adhere to the organization's published values.
