REGISTERED DIETITIAN

POSITION REQUIREMENTS:

  • Must be a Registered Dietitian in good standing.

  • Baccalaureate or advanced degree is required from an accredited college with major studies in food and nutrition or dietetics.

  • Have a minimum of one year's experience working with a frail or elderly population.

SUMMARY: AllCARE for Seniors is a Program of All-Inclusive Care for the Elderly (PACE), which is designed to provide community-based, comprehensive, integrated acute and long-term healthcare for eligible members of the frail elderly population.

The Registered Dietitian is a required member of the Interdisciplinary Team (IDT) and provides nutrition services for individuals enrolled in the agency's PACE program.

DUTIES AND RESPONSIBILITIES:

  • Comply with any State or Federal requirements for direct patient care, service delivery, participant rights, and the Quality Improvement (QI) activities and monitoring.

  • Comply with the agency's requirement for criminal background check.

  • Agree to abide by the philosophy, practices, and protocols of the PACE organization.

  • Participate in PACE Orientation.

  • Furnish only those services authorized by the PACE IDT.

  • Complete competency evaluations required for all staff that provide direct participant care.

  • Conduct initial and periodic assessments on PACE participants.

  • Participate in the development of the IDT Plan of Care and attend IDT meetings as required.

  • Communicate participant changes to team members.

  • Participate in the coordination of a 24-hour care delivery.

  • Complete all documentation in the medical record as required.

  • Provide nutrition counseling and education to participants/caregivers as needed and according to AND (Academy of Nutrition and Dietetics) standards.

  • Provide nutrition education to staff and the community.

  • Provide nutrition education to Congregate Nutrition Sites on at least a quarterly basis.

  • Provide nutrition counseling to all eligible home delivered meal clients, case management clients, and nutrition site members.

  • Work collaboratively with other team members implementing participant education.

  • Procures nutrition supplements and Adult Day Care (ADC) snacks per protocols.

  • Develops monthly ADC Snack Menus.

  • Reviews master agency lunch menu and verifies all requirements are met across all programs.

  • Coordinates with the agency's Nutrition Services Department on Home Delivered Meals and ADC meals.

  • Assists Center Manager in oversight of the PACE kitchen to meet regulatory requirements.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Ability to apply nutrition therapy principles to the senior population.

  • Ability to define problems, goals, and interventions for plans of care.

  • Knowledge of Quality Assurance principles and the ability to apply them to various situations.

  • Ability to set objectives and develop action plans.

  • Competent in identifying problems and developing effective solutions.

  • Strong critical thinking skills.

  • Orientation to detail and ability to see a broader picture.

  • Communicate effectively orally and in writing.
