The Telephonic Disease Management Program (TDM) assists Sutter Clinicians with the management of asthma, diabetes, heart failure, hypertension and hyperlipidemia. The Registered Dietician Case Manager (RD-CM) is responsible for the triage and coordination of services for patients enrolled in one or more TDM programs to facilitate optimal outcomes through collaboration with the client, the family, the physician and other members of the healthcare team.
The RD-CM also provides ongoing education and consultation to Case Managers regarding Medical Nutritional Therapy (MNT.) Communication with various departments and disciplines, telephone monitoring, patient assessment and education, development of care plans, and medication reconciliation or management are critical to success. The RD-CM works to support the patients in a professional manner consistent with existing and developing policies. The RD-CM consults with other members of the TDM Case Manager team, and provides consultation to team members regarding nutritional concerns.
Bachelor's degree in related field required.
Master's degree preferred.
Certified Diabetes Educator certificate from an AADE or ADA recognized program strongly preferred certificate in progress considered (Certified Diabetes Educator - CDE) preferred.
3-5 years 5 years of direct patient education experience with both type 1 and type 2 diabetes patients required.
0-2 years Experience with electronic health records and computerized patient care systems Epic experience highly recommended preferred.
0-2 years Motivational Interviewing or Shared Decision Making techniques preferred.
Skills and Knowledge
Strong familiarity with the etiology and current therapeutic modalities in the areas of diabetes care and cardiovascular disease
