The Regional Patient Care Manger assumes responsibility and accountability for clinical oversight, financial performance, and customer service for designated agencies in their assigned region. Collaborates with other team members to ensure that regulatory requirements are maintained.
Must have a thorough understanding of agency goals, objectives, policies, and procedures. Models and leads professional practices consistent with organizational goals of safety, customer satisfaction and quality. Supervises staff, provides clinical/ technical expertise, and engages in process improvements.
Assists in the fiscal budgeting and management process for the department including billing/revenue management, payroll, purchasing and supply cost management. Performs work assignments as needed to provide coverage and to mentor or assist staff. Interviews, hires, and develops team members with the goal of developing a highly effective work team. Engages in talent retention strategies for the area(s) of responsibility.
Responsible and accountable for the oversite of the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services, collaborates, and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs.
Requirements:
3 Years HH/HO/IV case management
3 Year Supervisory Experience
BSN
Licensure as a RN, Physicial or Occupational Therapist or Speech-Language Pathologist
Case Management certification based on specialty area required within one year of eligibility.
