Job Description Job Description Program Description/Summary: CCANO's Refugee Services Program provides holistic case management to support refugee self-sufficiency and integration to many clients a year.

The Case Manager II will provide mid-level professional case management work in a comprehensive purchase-of-service and community placement programs for Immigrants eligible for and social service programs. Job Duties: Manage a caseload of substantial size and complexity consisting of all refugee services clients requiring the utilization of all community agencies serving underprivileged immigrant populations with a need for specialized case management services; Assist less-experienced Case Managers by providing information concerning case management practices, administrative rules and regulations, community resources, vendors, and services.

Serve as a resource for other Case Managers Interviews clients, their families, and other responsible individual; assists in completing the application for services; collects basic data and obtains appropriate additional information from other agencies; and participates in social services team meetings to review cases Prepares and dictates case progress notes, social summaries, and other reports concerning the client's case Develop, in cooperation with other staff, an Individual Support Plan for each client Attends Social Services Team Meetings to discuss client cases in relation to eligibility, support plans, progress, and possible changes in the service program Prepare and arrange for clients to access services Monitors services to ensure that services are being fulfilled by the vendor, check on quality of services, and review client progress Request modifications for a non-productive service or enroll the client in an appropriate program Provides progress reports and counseling to clients, their families, and other responsible individuals Attends staff meetings concerning new or revised policies and procedures Visits services agencies, attends meetings, and confers with other Case Managers to obtain additional information concerning community resources for the Immigrant Community Conducts client assessments on a limited basis involving the use of specialized knowledge and applications associated with a specific discipline such as social work, psychology, special education, counseling, health care, or occupational therapy Prepares and maintains accurate expenditure records Reviews literature and confers with other Case Managers concerning theories and practices in the fields of social work, psychology, special education, and health care for the Immigrant population Exercises independent judgment and initiative in making decisions related to client services; receive general supervision from Program Manager Maintain availability to respond to emergencies; Performs other related work as assigned Can I be Successful? Do you possess the key skills which are considered essential for successful employment in this position? Key Skills:

Intermediate knowledge of case management methods, principles, and techniques.

Intermediate knowledge of immigrant and diverse populations and corresponding vendors and services available for clients. Intermediate knowledge of interviewing methods, principles, and techniques.

Introductory knowledge of the behavioral sciences and allied disciplines involved in the evaluation, care and training of Immigration and Migration. Ability to manage a caseload of clients with trauma induced behavioral patterns, keep support plans current, and maintain accurate records. Ability to develop a logical, feasible, and practical support plan for clients that will result in a proper self-sufficiency strategy.

Ability to collect analyze information to make decisions concerning a client's support plan. Ability to evaluate the progress of clients and the quality of their services programs. Ability to communicate effectively.

Do I qualify? Qualifications: A Bachelor's or higher level degree from an accredited college or university Bachelor's degree in Social Work or degree in a related field preferred; One or more years of professional experience as a member of an inter-disciplinary team assessing, developing, or implementing Individual Support Plans for specialized underprivileged population groups and/or Immigrant populations One year of related experience required as well as prior supervisory experience preferred, particularly in the areas of developing effective case plans, case management problem solving, values, attitudes and practices of case management, and monitoring case managers' performance; Bi-lingual (English-Spanish) candidates highly preferred. Proficiency in Arabic, Haitian-Creole, Burmese, or other languages valued; Ability to work with a culturally diverse population and sensitivity to the special needs of multi-cultural immigrant children, adults, and families; Ability to react to change productively and handle other essential tasks as assigned; Excellent oral and written communication and interpersonal skills, including maintaining effective work relationships; Strong management and organizational skills, and ability to work across all levels of management and staff; Proficiency in Microsoft Office Suite preferred.
