POSITION SUMMARY

The Referral Liaison is responsible for ensuring accurate processing of all referral requests by providers, clinical staff, and occasionally patients in a timely and professional manner. The Referral Liaison will represent Bellefonte Physician Services (BPS) within the medical community in a positive, courteous, and compassionate manner that will aid in the delivery of high quality health care to all patients.

Contributes to the success of the medical group by providing prompt, efficient, and friendly service via in person or over the phone. Performs a wide variety of tasks associated with referral tracking to be compliant with Patient Centered Medical Home.

JOB REQUIREMENTS
1.High School Diploma or GED equivalent.
2.3+ years prior employment experience in a clinic atmosphere or other healthcare setting is preferred.
3.Experience with clinical documentation is essential.
4.Experience with insurance carriers and prior authorizations is valuable.
5.Must be organized and able to manage competing priorities.

As a LPN with NeuroRestorative, you will have high-impact, providing nursing services, training and supports to address the medical and psychiatric needs of individuals with Acquired Brain Injury and other catastrophic injuries. Provide clinical care to address the medical needs of the individuals we serve Responsibilities: Monitor the health conditions of individuals served, ensure quality services provided * Provide prescribed medical treatment and personal care services * Administer medications and provide documentation according to established guidelines * Conduct assessments, implement health care plans and respond to changes of condition * Ensure physicians' orders are documented and carried out as prescribed * Serve as liaison and advocate with other medical and professional service providers * Provide input regarding health care services to be included in each Individual Service Plan (ISP) * Accompany individuals to medical visits in medical emergencies * Actively attend and participate in team meetings as required * Maintain confidentiality and treat others with dignity and respect in all you do Qualifications: What we ask of you: * Two year college degree in nursing or related field * One year of experience working a nursing capacity, ideally with medically complex patients * Current LPN/LVN licensure for state of residence * Current Basic Life Support (BLS), CPR, First Aid certifications; HIV/AIDS, BBP and ACT training * Ability to effectively communicate with staff, residents, and families What we offer you: * Flexible work schedules * Competitive pay rates; paid time off, Holiday Pay, and benefits for 30+ hour work week * Career development and advancement opportunities * Paid classroom and on-the-job training * A dynamic work environment where no day is ever the same as the next We have a great work environment and you'll be surrounded with a strong and supportive team of peers, leaders and colleagues.
