Position Summary

The Referral Intake Program Manager is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Referral Intake Program Manager plays a pivotal role in managing the referral portal system for home health services. This position involves overseeing the day-to-day administration of the portal, analyzing data to identify opportunities for improvement, and ensuring the portal's details are optimized and maintained to support efficient referral processes. This position will be responsible for optimizing the intake processes, managing the referral portal, and overseeing intraday reporting to drive operational efficiency and excellence in patient care delivery.

Position Specific Responsibilities

  • Develops and implements best practices for home health intake processes related to referral portals to ensure efficient and effective patient onboarding.

  • Leads process improvement initiatives to streamline intake workflows, reduce inefficiencies, and enhance the overall patient and user experience.

  • Collaborates with cross-functional teams, including clinical, administrative, and IT departments, to align intake processes with organizational goals and standards.

  • Monitors key performance metrics related to intake processes and identify opportunities for optimization and enhancement.

  • Serves as the primary administrator for our home health referral portal, overseeing user access, permissions, and system configuration.

  • Ensures the accuracy and integrity of referral portal data and content, including patient information, provider profiles, and service offerings.

  • Provides training and support to users on referral portal functionality and best practices.

  • Assesses and improves the usability and functionality of the referral portal to meet the needs of internal stakeholders and external partners. Develops and maintains intraday reporting processes to monitor key operational metrics and performance indicators.

  • Generates regular reports and dashboards to track patient intake volumes, referral trends, and other relevant metrics.

  • Analyzes report findings to identify areas for improvement and make data-driven recommendations to enhance operational efficiency and quality of care.

  • Collaborates with leadership and operational teams to implement action plans based on report insights and drive continuous improvement initiatives.

  • Performs other duties as assigned.

Education and/or Experience

  • Bachelor's degree in healthcare administration, business analytics, or a related field desired.

  • Strong proficiency in data analysis tools and techniques, such as Excel, PowerBI, or Tableau is preferred

  • Knowledge of healthcare regulations and compliance requirements related to referral management is a plus.

  • Experience administering and managing referral portal systems in a healthcare setting preferred.

  • Experience working in home health or a related healthcare field is preferred.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.

  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications as well as excellent attention to detail and organizational skills. Ability to communicate effectively with stakeholders at all levels of the organization.

  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
