Referral coordination is a critical component in providing appropriate care and follow-up to patients requiring additional services. The Referral Coordinator will facilitate the referral process, including communication and assistance for the patient, tracking of referrals, and maintaining accurate documentation of the referral within the patient's medical record.
KEY RESPONSIBILITIES
Tracks and documents activity related to referrals in the Electronic Medical Record
Runs and monitors referral reports
Assists patients in all aspects of facilitating the referral
Aids in tracking and administering contract performance guarantees in partnership with sales, account management, legal and reporting
Facilitates the sending of information to service/referral providers; Follows up with service/referral provider regarding results
Communicates with provider/nurse care teams regarding referral orders
JOB REQUIREMENTS
Education:
Graduate of accredited school of medical assisting, or combination of equivalent education and experience
Some client sites require an LPN and, in that case, will need to be a graduate from an approved nursing program with an associate degree or equivalent.
Experience:
Minimum one (1) year experience in referral coordination
Outpatient care experience preferred
Knowledge, Skills & Abilities:
Excellent communication skills; both written and verbal, with proven organizational and facilitation ability.
Strong team player with ability to be a self-starter professional who can work independently and proactively toward program improvement.
Proficient in Microsoft Office including Word, Outlook, and Excel required
Outstanding leadership, organizational, planning and execution skills
Skill in understanding the necessity and to preserve and maintain confidential communications and legally privileged information
Effective positive human relations skills including tact, diplomacy, and collaborates with all levels of staff and management.
Work successfully with diverse populations and demonstrated commitment to promote and enhance diversity and inclusion
PHYSICAL DEMANDS
Manual Dexterity: Consistently operates a computer and other office equipment
Physical Effort: This is largely a sedentary role; however, will require the ability to walk, bend, reach or stand as necessary. Work requires handling light-weight materials or equipment up to 25 pounds
Working Conditions: Ordinary office work environment
Hazards: Ergonomics. Little to no exposure to hazards
