Compensation (Pay): $16.25 to $23.56/hourly, based on experience.

Summary: The Referral and Resource Navigator's primary role is to assist identified patients with identifying barriers to effective care, assisting the patient in working with community resources to overcome these barriers and is key in obtaining medically appropriate services through referral processing inside and outside of the Peak Vista Community Health Centers. Employee demonstrates knowledge of and adherence to the Compliance Plan and conflict of interest requirements. The employee enhances the effectiveness; efficiency and productivity of the department by contributing to, and participating in, departmental performance improvement goals

Qualifications:

Education:
High school diploma or equivalent education.
Work Experience:
Minimum 1-year experience in a healthcare or social service setting is required.
Computer Skills: Basic - ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co-workers, leadership, and other departments; enter and correct data, modify a workbook, format a worksheet, and use printing functions; create a simple presentation in PowerPoint, run it, and print it
Certificates and Licenses: CPR Certification to be obtained within the first 90 days of employment.
