Position Summary:

Recovery Support Navigator (RSN) is a paraprofessional who provides care management and system navigation supports to Members with a diagnosis of substance use disorder and/or co-occurring mental health disorders. The purpose of RSN services is to engage Members as they present in the treatment system and support them in accessing treatment services and community resources. The RSN is responsible for providing outreach case management services and coordination of care to a clinically diverse population.

Position Requirements:

  • Bachelor's Degree in psychology, social work or related human services field is required. Must be able and willing to transport persons served in personal vehicle.

    As such, must have a valid driver's license, reliable vehicle, and ability to satisfy LHBS approved driver policy. Travel and an ability to meet persons served in a variety of outreach settings is required.

  • Training or relevant work or lived experience in the areas of mental health and/or addiction/recovery preferred. Experience delivering community based services is a plus. Certificates, Licenses, Registrations, LADC/CADC preferred.

  • Demonstrates good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from mental illness and/or substance use, as applicable. Demonstrates the ability to evaluate what is needed by each individual and adjust approach accordingly.

    Have in-depth knowledge of local resources. Experience with accessing resources and navigating mental health and/or substance abuse treatment systems a priority. Demonstrates excellent communication, documentation, time management and organizational skills.

VIEW JOBS 10/9/2019 12:00:00 AM 2020-01-07T00:00 The CATALYST Program and Project RECOVER are seeking a highly motivated individual for the position of Recovery Support Navigator. Under the supervision of the program manager, this position helps patients navigate their addiction treatment and recovery and provide integral support to program operations. Key duties include referral and linkage to resources, troubleshooting barriers to care, answering patient communications/scheduling appointments and follow up as needed. The CATALYST Clinic is multidisciplinary team striving to provide wrap-around comprehensive care for youth and young adults and who use substances. Project RECOVER at Boston Medical Center utilizes Peer Recovery Coaches (PRC) to meet individuals at two local detoxes to facilitate engagement with long-term addiction treatment after discharge. ESSENTIAL RESPONSIBILITIES / DUTIES: Assist patients in referring/connecting to services and resources including addiction treatment, recovery support, healthcare and social supports such as job training, GED programs, housing, financial resources and food access Answer phone for referrals, appointments, patient needs and triage as appropriate Visit clinical sites in hospital to provide administrative support in scheduling appointments with patients, provide information to patient, obtain and verify contact information/consents, assist with care plan, etc. Coordinates/removes barriers to transportation to appointments and services as needed Assists in resolving insurance issues and obtaining authorizations as needed Works with external referral sites to ensure proper referrals to and from CATALYST Helps patients/families follow up with their medical appointments through phone call reminders. Works with patients to provide the medical team with necessary information to help them care for them Helps patients/families solve daily problems that interfere with care Helps patients/families interact with community-based resources and services. Follows up with patients who missed appointments or have fallen out of care Maintain supply of programmatic forms including copy, print & laminate materials, as needed Works collaboratively with the CATALYST and RECOVER teams Maintain patient confidentiality Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best patient care may be provided Performs other duties as assigned or necessary JOB REQUIREMENTS EDUCATION: Bachelor's degree preferred. CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED: None EXPERIENCE: Prior addiction or behavioral health clinical/program experience helpful. KNOWLEDGE AND SKILLS: Knowledge of addiction and addiction treatment, health care systems, and community resources Empathy and understanding of persons with addiction, behavioral health disorders and trauma Proficiency with standard software programs used in healthcare (such as MS Word, Excel, Outlook) and web browsers, patient registration systems and electronic medical record. Exceptional organizational skills required. Ability to multi-task and work under time pressures. Ability to communicate well both verbally and in writing with other members of the clinical team. Excellent English communication skills. Additional language skills appropriate to the patient population served (especially Spanish and/or Creole) is desirable. Self-directed and independent.
