Flexible, combination of weekend/evening and weekdays.

Position split between Salem Emergency Services- Sat/Sun 4pm to 12 am and the Beverly Emergency Department, Addison Gilbert Hospital, and outreach in the community- 3 weekdays 8:30 am to 4:30 pm.

40 hours per week, full time.

Responsibilities

Meet with individuals who present in an ED post-overdose and act as a resource for any individual who presents to the ED seeking services for a substance use disorder.

Be knowledgeable about the substance use disorder service system and link the individual to treatment and recovery resources including but not limited to Acute Treatment Services, sober housing, benefits, and Narcan education.

Act as an advocate for the needs of the individual

Provide education on the signs and symptoms of an overdose, overdose prevention and the use of naloxone to the individual and offer the individual an overdose prevention kit.

Provide short-term telephonic (or text) or in-person follow-up support, coaching, and assistance to help the individual access treatment or recovery support services. Obtain the phone number of the individual, if possible, and give the individual his/her contact in order to follow-up the next day.

Conduct a warm handoff of the individual to appropriate treatment or recovery services including a Community Support Program (CSP) to encourage continued treatment and recovery support in the community

Demonstrate an understanding and belief in the recovery process and Recovery Oriented Systems of Care Model

Knowledge of basic crisis intervention, Motivational interviewing, and case management techniques preferred.

Demonstrates good boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from a substance use disorder.

Demonstrates the ability to evaluate what is needed by each individual and adjust approach accordingly.

Have in-depth knowledge of local resources.

Provides temporary assistance with transportation as needed to essential behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.).

Refer the individual to Intensive Case Management Program (ICM) if within their health insurance benefit.

Complete training provided by agency and participates in ongoing supervision by a staff person who is also trained and as a Recovery Coach and has a greater length of experience than the supervisee as a Recovery Coach;

Ensures that all written and billing documentation is complete in a timely manner.

Manages CHART Tasks by patient and updates care manager tool (for CHART work only)

Other related duties as assigned.

T-About the Job

A member of the ESP team to provide non-clinical support to individuals who present in an ED post-overdose. Serve as a recovery guide and role model in the management of recovery and assist the individual to identify and overcome barriers to recovery, connect the individual with recovery support services and encourage hope, optimism and health.

Provide support with problem solving and advocacy to assist individuals to meet their recovery goals. Provide education to individuals and family members on the recovery process. As a member of the CHART program, this position engages with patients who are high ED utilizers to coordinate with peer networks/natural supports, coordinate referrals to community social services providers, follow up on referrals and contribute to the treatment plan revisions.

Qualifications

  • 18 years or older and high school diploma required; bachelor degree preferred.

  • Driver's license and car required.

  • Be a Peer with lived addiction experience.

  • Have at least two years of current, sustained recovery without reoccurrence.

  • Complete the Bureau of Substance Abuse Services (BSAS) Recovery Coach Academy or the Connecticut Community Addiction Recovery (CCAR) training prior to working with individuals in the ED and within one month of hire. (for ESP work only)

Physical Requirements

While performing the duties of this job, the employee is regularly required to walk; sit; use hands to finger, handle, or feel; and talk or hear. Sitting or standing for long periods of time, typical of an office or emergency room is also required.

The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, and ability to adjust focus.

Must be able and willing to transport persons served. As such, must have a valid driver's license, good driving record, and reliable vehicle. Travel and an ability to meet persons served in a variety of outreach settings are required.

The Recovery Coach (RC) provides emotional and social support to individuals currently in recovery. If applicable, RCs must be actively engaged in their own personal recovery and be able to share real-world knowledge and experiences with others who are on their own paths to recovery. Willingness to share lived experiences with others is imperative to establishing productive relationships with the individuals being supported. The focus of the role is to create supportive relationships that are non-clinical and focused on developing goals and removing obstacles to recovery. The RC will link the individuals being supported with the recovery community and other social services As a team member at our health center, you will work with staff who are racially, ethnically and culturally diverse, so being culturally competent is required. A sense of humor is also very important! Responsibilities * Provide emotional and social support to individuals in recovery. * Share lived experiences and use coaching and mentoring techniques to support the assigned individual(s). * Help assigned individual(s) in making positive life changes and developing skills to facilitate recovery. * Discuss with assigned individual(s) new strategies for developing recovery-supportive friendships, reconnecting or improving family relationships, and using recovery community services. * Act as a role model in an open and transparent manner. * Use the peer relationship, when appropriate, to assist assigned individual(s) with motivation. Facilitate connections to primary and specialty medical care, as well as social services (such as applying for benefits and navigating relevant systems). * Provide linguistically appropriate and culturally sensitive support, embracing the diversity (e.g. racial, ethnic, gender/gender identity, sexual orientation, physical and intellectual challenges), of supported individual(s). * Act as a mentor, assisting individual(s) with their recovery process and supporting their goals and decisions as well as developing and enacting a Wellness Plan. * Link individuals to transportation resources as well as providing transportation support when able to do so. * Connecting and delivering services in a setting that is safe for both the RC and assigned individual(s), such as the community health center, local coffee shop, local park, assigned individual(s) home, appropriate self-help meetings, recovery support center or the home of the assigned individual(s). * The RC must act as a recovery liaison and support the assigned individual(s) in preparing for or accompanying them to meetings with, for example, probation officers, social workers, and child protection/child welfare workers. * The RC must assist assigned individual(s) in creating personally meaningful links to treatment, peer recovery support services, and mutually aid and support them in their efforts to build their capacity to move between and among these services and supports as need (e.g. ATS, residential programs, 12 Step Meetings, Sober Homes, etc.). * The RC must serve as an advocate for assigned individual(s) and assist them in learning self-advocacy skills. * The RC must provide Recovery Coaching services in a person-centered and strength-based manner. * Participate on multi-disciplinary care teams to reflect the recovery lens. * Develop relationships with community based stakeholders (e.g. police and fire departments, emergency departments, and courts) to support outreach and engagement with potential patients. * Perform follow-up activities with patients who may have dropped out of care. * Support access to higher levels of care when needed (eg, ATS, residential, etc). * Document encounters in the health center's electronic medical record (EMR) system. QUALIFICATIONS * Minimum of high school diploma or GED. * Certificate indicating successful completion of training at the DPH/BSAS Recovery Coach Academy, or similar training approved by the State of Massachusetts and which also satisfies MassHealth reimbursement criteria. * Candidate must be a Certified Addiction Recovery Coach (CARC) or provide evidence he/she is working toward a certification. * Participation in trainings and/or coursework designed to prepare the RC for various duties. * Preference will be given to bilingual/bicultural individuals who are Spanish or Portuguese.
