About the Job
Lahey MassHealth ACO (LMH-ACO) is responsible for improving coordination of patient care, reducing unnecessary hospitalizations and readmissions, emergency department visits and revisits, and reducing total cost of care over time. LMH-ACO will provide services in collaboration with contracted Managed Care Organizations as well as Community Partners for Behavioral Health and Long Term Services and Supports, as appropriate.
The Recovery Coach, LMH-ACO, provides non-clinical support to individuals with substance use disorder (SUD), including but not limited to connecting the individual with recovery support services, serving as a recovery guide and role model in the management of recovery, assisting the individual to identify and overcome barriers to recovery, providing support with problem solving and advocacy, and educating the individual and family members on the recovery process.
Education: High school diploma required; bachelor degree preferred
Peer with lived addiction experience
At least 2 years of current and sustained recovery without reoccurrence
Proficient in computer skills
Information technology and electronic medical record experience preferred
Experience with MassHealth population preferred
