Summary:

Implements and maintains best practices for the provision of care to the maternal child population, utilizing a family-centered, culturally sensitive approach to provide support and assistance.

Responsibilities:

  • Assesses the physical, emotional, socio-economic needs and environmental needs of the MCH population and their families.

  • Assesses the developmental needs of the patients served and refers to appropriate community services.

  • Identifies problems utilizing the nursing process, interdisciplinary coordination, and physician's treatment plan.

  • Develops a plan of care with the patient and/or responsible caregiver and physician to meet the needs of the patient.

  • Ensures the maintenance of safe, sanitary, and developmentally appropriate environments for the MCH population.

  • Provides post-partum visits to include assessment of the mother and baby, breastfeeding support, and evaluates family adjustment.

  • Provides education to the patient and/or caregiver regarding prevention, treatment, and care of condition.

  • Adheres to the mandatory abuse and neglect reporter guidelines.

  • Responsibility for supervisor of the services rendered to the patient and family by the homemaker/home health aide, according to State and Federal guidelines and Joint Commission standards, and submission of periodic evaluation of such to the Home Health Aide Supervisor.

  • May visit and complete an assessment of assigned patients receiving homemaker/home health aide services as often as necessary, based on the patient's condition, but not less than every two (2) weeks for patients receiving more than homemaker/home health aide services, and not less frequent then every four (4) weeks for patients receiving homemaker/home health aide only.

  • Coordinates services with others involved in the care of the patient.

  • Fulfills all compliance responsibilities related to the position.

  • Performs other duties as assigned.

Position Summary The Medical Office Receptionist is typically a PACT team member in the front office reception area. This position schedules clinic appointments, in accordance with VA guidelines. The Patient Services Specialist also prepares and compiles records in the clinic. This position supports the clinical staff by answering phones, welcoming patients, and addressing questions. As a team member, the Patient Services Specialist collaborates with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community based services involved in developing the patient care plan. Core Responsibilities * Actively assists with the administrative support related to patientcare. * Inputs all patients' information into VISTA/CPRS. * Verifies any and all clinical reminders "due" at the time of each patient visit are completed prior to check out. * Check patients in and/or out of the clinic. * Schedule clinic appointments. * Answers phones and timely relays messages. * Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items. * Remain focused on achieving excellent clinical outcomes through data input accuracy and within the specified VA guidelines.• Must provide excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors. * Participate in the clinic's outreach events to help support the clinic's enrollment initiatives. * Support patient enrollment and retention in appointments, follow-up visits or nurse visits. * Embrace and support new initiatives, whether clinical or operational. Requirements Qualifications * High School Diploma/GED or equivalent education * Strong computer skills, EMR experience preferred * Demonstrated high quality customer service & organization skills * Minimum, 1-year experience in a clinical or call center environment (preferred). * Must be credentialed and remain in good standing through the Veterans Health Administration (VA). * Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency - AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract * Proficiency in written and spoken English. * Energetic and optimistic demeanor * This is considered a safety sensitive position.
