Job Purpose:The Certified Diabetes Educator is responsible for providing diabetes education and assists with medical management of patients per physician approval. The CDE will work collaboratively with team members of the Diabetes Program, physicians, physician extenders, and other USMD staff to provide excellent patient care. The various programs include: Type 1, Type 2, GDM, Pre-DM and Insulin Pump Therapy. The American Association of Diabetes Educators (AADE 7 Self-Care Behaviors framework) is incorporated in these programs. The CDE is responsible for training patient on blood glucose monitors, continuous glucose monitors and insulin pumps is preferable but not required upon hire. Must be proficient in Medical Nutrition Therapy for Diabetes primarily and be able to educate in other diseases.
Education & License Required: Certified Diabetes Educator- current certificate required/NCBDE; Registered and Licensed Dietician
Proficient knowledge of Diabetes disease pathophysiology, Experience related to patient education, motivational interviewing skills and self-management goal setting. Excellent verbal and written skills. Proficient computer skills, including Microsoft products. BLS current.
Work Experience: Two or more years' post-licensure experience in a clinical setting
Essential Job Responsibilities:
Provides diabetes and nutrition education one-on-one and class session per PCP referrals for Type 1, Type 2, Gestational Diabetes and Insulin Pump Therapy- Advanced Carbohydrate Counting (preferred).
Utilizes ADA and AADE approved curriculum and education materials for patient instruction.
Utilizes appropriate motivational interviewing techniques necessary for coaching and assisting the patient to complete self-management goal/action plans.
Instructs diabetic patients on proper usage of insulin-name, dosing, peak, onset, duration and side effects of type of insulin prescribed by PCP/Endocrinologist.
Instructs diabetic patients on proper usage of GLP-1 name, dosing, onset, duration and side effects. These are prescribed by PCP/Endocrinologist.
The RD/LD, CDE is responsible for training patients on diabetes medical devices: Blood Glucose Monitors is required, continuous glucose monitors and insulin pumps is preferred but not required at time of hire.
Enters timely and accurate data into the EMR and other applications necessary to communicate patient needs and to ensure complete documentation of patient visits and phone calls.
Maintains current knowledge in regards to diabetes as well as treatments and medications to condition
Provides diabetes education and counseling to patients and their family members in an individual and/or group setting utilizing the education curriculum developed according to the National Standards from the ADA and AADE.
Utilizing appropriate motivational interviewing techniques necessary for coaching/counseling and assisting the patient to complete self-management goal/action plans.
Provides education on medical devices which include: Blood glucose monitoring is required, insulin pumps, continuous glucose monitors and other insulin delivery systems is preferred but not required at time of hire.
Serves as a resource or consultant for team members and USMD Physicians.
