Job Summary:

The primary role of the Rapid Rehousing Team Manager is to assist clients in attaining housing rapidly and maintaining safe and stable housing long term. In addition, this position will work with clients to increase income, ensure health insurance coverage, and help those who are able to work secure employment. The Rapid Rehousing Team Manager will provide support, advocacy, safety planning, and counseling to individuals experiencing homelessness.

Experience, Knowledge, Skills:

Minimum Requirements

  • Bachelor's degree from an accredited college or university in Counseling, Social Work, Human Services, or a closely related field

  • Five (5) years mental/behavioral health direct care experience.

  • Two (2) years of supervisory experience.

  • Two (2) years of experience in managing rehousing for people experiencing homeless

  • Two (2) years experience working with and servicing people with very complex needs, including but not limited to chronic homelessness, serve trauma, substance use disorders, mental illness.

  • Possession of a valid driver's license.

OR

Any combination of related education, experience, certifications and licenses that will result in a candidate successfully performing the essential functions of the job.

Preferred

  • Bilingual in Spanish and English.

  • Master's degree in support services or related field of study.

  • Six (6)+ months of experience in Oracle HCM.

  • Six (6)+ months of experience working within a municipality.

  • Three (3) years' experience in managing rehousing for people experiencing homeless

  • Three (3) years' experience working with and servicing people with very complex needs, including but not limited to chronic homelessness, serve trauma, substance use disorders, mental illness.

  • One (1) year of Housing First Services experience.

  • Ability to work independently with minimal supervision and reliable professional judgment, as well as collaboratively with a team to achieve desired results.

Travel:

  • Approximately 20% - 50% travel is required.

Essential Functions:

  • Completes coordinated intakes, assessments, and screenings to determine housing eligibility based on HUD and Colorado State Department of Housing (DOH) guidelines.

  • Very familiar with HMIS and understands the need to have strong data to tell our stories.

  • Completes strengths-based housing plans that identify barriers to housing and set a path to housing stability; reviewed and updated monthly.

  • Supervisors Case managers to provide case management services focused on housing, increasing income, establishing and/or expand family and community-based supports, and assisting in increasing access to mainstream services.

  • Develops a relationship within the homeless service system.

  • Assists in program development and quality assurance activities as directed by Homeless Solutions Director.

  • Supervisors Case managers with locating available housing units, the leasing process, landlord mediation, and tenant/landlord rights and responsibilities.

  • Serves as primary contact for participant enrollment; responsible for determining program. eligibility and funding source, and assigning participants to Rapid Rehousing Case Managers

  • Assesses the need for crisis intervention counseling and/or advocacy.

  • Attends Homeless Solutions trainings and workshops related to housing.

  • Attends monthly case management meetings, meetings with supervisors and staff meetings as scheduled.

  • Cultivates new and existing partnerships in Greeley and the surrounding area that will increase client skills and establish goals (life management skills, educational and/or career goals, financial management skills, health and wellness goals and decision making skills).

  • Responsible for reading all logs/emails in a timely manner to ensure you are up to date with current information pertaining to the residents.

  • Prepares concise, accurate, and timely reports for the Homeless Solutions Director and Assistant City Manager on progress towards established goals.

  • Manages RRH team to ensure efforts are in line with established performance goals, accountability/compliance expectations, and long-term program goals.

  • Provides direct supervision of team of Housing Stability Case Managers.

  • Regularly evaluates programs for effectiveness, including identification of opportunities for continued improvement.

  • Performs other duties, as assigned.

Work Environment and Physical Requirements:

  • Work is primarily performed in an office environment.

  • The City of Greeley has established a formal telework policy that allows eligible positions, in coordination with their Departments' management team, an opportunity to incorporate a component of remote work into their working schedule. Physical Requirements:

  • Specific vision abilities required by this job include close vision, peripheral vision, depth perception, and ability to adjust focus.

  • Vision enough to read computer keyboards, reference books and other written documents.

  • Communication skills sufficient to convey information to the public via telephone and in person.

  • Manual dexterity sufficient to accurately input, retrieve and verify work assignments.
