This position is full-time Monday through Friday and requires you to have a current RN license.

Examples of Duties Performed:

  • The Resident Assessment Instrument (RAI) Director is responsible for the development and completion of patient assessments in accordance with federal and state requirements and organizational goals and guidelines.

  • Conducts and/or coordinates patient and family assessment interviews and develops care plans consistent with patient clinical needs, risk areas, and wishes.

  • Schedules and coordinates meetings with family members and interdisciplinary team members to discuss patient care plans, special needs, and/or appropriate interventions.

  • Performs periodic patient physical assessments and monitors the Minimum Data Set (MDS) process for accuracy.

Requirements

To be considered for this opportunity applicants must:

  • Have a current license as a Registered Nurse from the Georgia State Nursing Board and demonstrated knowledge of data entry, MDS coding, and entire RAI process.

  • Have at least one year of supervisory experience in a hospital, long-term care facility or other related health care field.

  • Embrace the standards, values, and beliefs of the organization.

  • Possess ability to communicate effectively both orally and in writing.

  • Be capable of maintaining effective working relationships with patients, family members, department heads, and fellow associates.

VIEW JOBS 10/28/2018 12:00:00 AM 2019-01-26T00:00 POSITION ANNOUNCEMENT October 26, 2018 POSITION: GEAR UP Project Director LOCATION: Albany High Complex DESCRIPTION: The Gaining Early Awareness & Readiness for Undergraduate Programs (GEAR UP) Project Director will develop and implement all projects and plans for the successful implementation of the school system's federal GEAR UP grant. GEAR UP Project for the Dougherty County School System (GUDC), is designed to serve a cohort of approximately 2100 students beginning with 6th and 7th grade students enrolled in the four DCSS middle schools during the 2018- 2019 school year. Through the provision of the grant, the cohort of students will be served for seven years. The GUDC project includes the following objectives: 1) Increase GUDC students' academic performance and preparation for postsecondary education. 2) Increase GUDC students' high school graduation rate and participation in postsecondary education. 3) Increase educational expectations of GUDC students and increase student and family knowledge of postsecondary options. The GEAR UP Project Director will develop and implement all projects and plans for the successful implementation of the school system's federal GEAR UP grant. PERFORMANCE RESPONSIBILITIES: * Oversees all projects and plans necessary for the successful implementation of the school system's GEAR UP grant. * Monitors and validates effective use of GEAR UP funds and effectiveness of the GEAR UP grant local implementation. * Maintains open and active collaboration between the school system and community partners, actively representing the school system on boards, committees and at meetings. * Supervises GEAR UP Coordinator and Staff Assistant and ensures that staff are meeting or exceeding all job expectations. * Plans, facilitates, and evaluates GEAR UP Advisory Board and Community Partner meetings as required. * Works with school system administration to ensure on-going progress toward identified goals and objectives. * Meets grant evaluation requirements by working with project evaluators to regularly collect and analyze data to determine progress toward identified goals and objectives. * Serves as the district contact for GEAR UP implementation. * Provides quality technical expertise to support all areas of the initiative, particularly teacher and leader effectiveness measures. * Facilitates effective communication to staff, students, community, and grant partners. * Is available to attend all Board of Education meetings and other meetings as required or requested. * Maintains records, meets all federal timelines and requirements, and make reports as directed. * Maintains clear and timely communication relative to all areas of assigned responsibility. * Perform other duties as assigned by the Principal.
