Now hiring a Quality Program Manager-Population Health

This opportunity is open to remote applicants in the United States, with the exception of the following states:

Wyoming, North Dakota, and Ohio. Must be able to work mountain time

Anticipates, develops, and implements complex, multi-departmental initiatives supporting PHP's population health management strategy, including targeted components of our wellness and health education framework. Role includes implementation and oversight of population health initiatives, often in partnership with providers and community stakeholders, execution of population health focused Member digital engagement strategies, and implementation of new clinical and population health programs and partnerships. Works with population health teams to obtain and maintain related accreditations, meet current regulatory requirements, and plan for emerging regulatory requirements.

Qualifications

  • Bachelors degree in Business, Public Health, Nursing, Healthcare, or related field, Masters preferred.

  • 2 years professional experience with 1 year healthcare preferred

  • Certified Health Education Specialist (CHES) qualification, preferred.

  • Experience with communicating and presenting to leadership groups, committees, and teams, preferred.

Responsibilities

  • Develop and analyze reports, program plans and milestones

  • Utilizing quality tools, identifies, develops, builds, and initiates evidence based programs for improving health outcomes for specifically delineated programs or populations

  • Creates and updates work plans, tasks, timelines, and measurements.

  • Selects article topics and reviews draft articles for providers and members

  • Works with identified stakeholders regarding compliance, including current and emerging

accreditation, reporting, and regulatory requirements related to population health, including health equity, social determinants of health, health literacy, and culturally and linguistically appropriate services.

  • Performs outcomes assessments *Identifies & resolves issues, escalates as necessary

  • Facilitates collaboration throughout the health plan and external organizations, to optimize the success of population health management activities using input and resources from the appropriate business units

  • Negotiates with vendors and collaborates with contracted network providers, various coalitions, community organizations, etc.

  • Utilizes organization s strategic plans to develop, monitor, and report program plans and milestones.

  • Recognizes change in the managed care environment and develops responsive initiatives

  • Compiles and presents summaries of health management activities in a professional format to the appropriate internal or ext
