Job Description

Supports the HHCMG Quality and Safety Department in conducting ongoing performance improvement activities for the designated site and/or region of care. We are currently hiring to support HHCMG offices in the Western part of CT from the North Western boarder to the shoreline.

Approximately 30% of the time is spent in the field visiting offices. Home base office is in Wethersfield, CT.

Leads performance/process improvement for targeted initiatives, oversight of collection/abstraction and aggregation of data, and the analysis of data /metrics to assess performance.

Collaborates with key stakeholders/ departments in developing best practice standards to ensure the reliable, safe design of care processes and workflows.

Colleague is a subject matter expert in ambulatory quality and safety standards, improvement science and high-reliability concepts.

Core to this role is functioning as a consultant to ensure compliance with all required agencies. Works closely with clinical teams and medical staff to develop practices, peer review, quality, and safety efforts aligned with HHC's core values.

Qualifications

Registered Nurse or other clinical healthcare professionals, such as technologist, paramedic, EMT.

Bachelor's degree in Nursing, Health Management or related discipline preferred.

If non-clinically trained, must have a Master's degree in related field such as MHA or MPH

Minimum of 5 years of experience in health care setting required.

Experience with database/spreadsheets required.

Experience with Quality Improvement tools and techniques, regulatory experience preferred.

CPHQ, CPPS or other quality certification preferred.
