Description of Work

This position is critical to the facility's compliance with Certified Medicare Services (CMS) Long Term Care regulations. The position assists with development, implementation and evaluation of the facility's Quality Assurance/Performance (QAPI) Program. A primary function of this position is providing oversight of the facility's Patient Safety Event Reporting System. This position reports to the Director of Standards Management. Responsibilities include but are not are not limited to:

  • Facilitate and monitor implementation of continuous quality improvement efforts at the facility.

  • Provide oversight of the Patient Safety Event Reporting System (RL6) to include review, analysis and follow-up of resident safety events.

  • Conduct event investigations and develop action plans.

  • Conduct Root Cause Analysis (RCA) for high severity events.

  • Prepare statistical and narrative reports for submission to the facility's Quality Improvement Committee.

  • Collect, analyze data and provide monthly reports to the facility's Performance Improvement Project Teams.

  • Provide training and support to the facility staff on the use of the Event Reporting System.

  • Participate in implementation of the CMS LTC regulations.

  • Collaborate with supervisor and other disciplines regarding quality improvement projects/initiatives.

Work schedule: 40 hours per week; Mon

  • Fri 8:00am

  • 4:30pm

Longleaf Neuro Medical Treatment Center (LNMTC) is a certified Medicare and Medicaid facility nursing home that provides a continuum of care to both adults with severe and persistent mental illness that also have long-term medical conditions requiring residential, medical and nursing care, and to adults with a diagnosis of Alzheimer's or other related dementia. LNMTC facilitates high quality skilled and intermediate nursing care throughout four Nursing Unit Floors and an Alzheimer's Unit that operates 24 hours seven days a week, with 1st, 2nd and 3rd shifts.

  • Excellent oral and written communication skills.

  • Knowledge of and expertise interpreting quality standards, policies and procedures based on all applicable state/federal regulations, statues and DHHS Departmental procedures.

  • Experience planning, organizing and implementing a continuous quality improvement process with multi-disciplinary groups.

  • Excellent problem solving and analytical skills.

  • Experience using a computer; proficient in Microsoft Word, Excel and Powerpoint.

  • Knowledge of QA principles, experience with data analysis and use of QA tools.

Management Prefers candidates with the following:

  • NC licensure as a Registered Nurse

  • Work experience in Quality Assurance/Performance Improvement/Patient Safety

  • Experience managing an electronic event reporting system such as RL6

  • Knowledge of CMS Long Term Care Standards

Minimum Education and Experience Requirements

Master's degree in psychology, social work, education, health, business administration or related human service field from an appropriately accredited institution and two years of experience in professional level treatment programming for the applicable client population

OR

Bachelor's degree in one of the fields listed above from an appropriately accredited institution and three years of experience as indicated above

OR

An equivalent combination of education and experience.

Degrees must be from appropriately accredited colleges or universities.
