Purpose of Job:
Improve the overall health of the communities we serve by facilitating clinic or department level improvement efforts as follows:
Essential Duties and Responsibilities:
o Analyzing and displaying data.
o Facilitating clinic/department leadership meetings.
o Leading staff in change management.
Acts as a systems and data resource to colleagues.
Maintains general knowledge of all clinic/department processes.
Works effectively with QI team to share and spread best practices.
Aligns clinic/department quality improvement efforts to improve population health.
Validates end user reports as needed.
Performs other duties as assigned.
Education/Experience: Work history or education that demonstrates experience utilizing data to teach, effect change, solve problems and foster analytical thinking. Previous experience working for underserved populations preferred.
Skills:Ability to work proactively and collaboratively with people at all levels of the organization. Ability to work independently and part of a team as required. Intermediate or advanced level of expertise with Microsoft Office tools preferred. Electronic health, pharmaceutical or dental records system and/or quality improvement experience preferred.
Required to stand, sit and be mobile up to one-third of the time. Required to use hands to finger, handle or feel over two-thirds of the time; while reaching with hands and arms occurs one-third of the day. Required to read from text and computer screen over two-thirds of the time. Climbing or balancing, stooping, kneeling or crouching occurs less than one-third of the time. Communicating occurs constantly throughout the day. Lifting occurs about half the time up to 10 lbs and less than one-third of the day from 25-40 lbs. Rarely is there a need to lift more than 41 lbs.
