We're looking for someone to join our team as a Quality Care Coordinator Supervisor who:

The Quality Care Coordinator (QCC) Supervisor is responsible for the oversight of all assigned staff in their daily duties to maintain and improve population health for the patients of Clinica Sierra Vista. The responsibilities of assigned staff include but are not limited to patient outreach and education, data reconciliation and analysis, and other related tasks. The primary duties of the QCC Supervisor shall consist of monitoring and tracking the performance of clinical quality measures (HEDIS, UDS, P4P), operational and conceptual problem-solving, building partnerships, and project planning, development, implementation, and monitoring. This position works under the guidance of the Quality Improvement Manager of Quality & Population Health Management in the absence of the QI Manager on projects using data available from CSV's EMR (EPIC) and/or external sources. The Quality Care Coordinator Supervisor will participate in PDSA's internally and externally to drive process improvement efforts across all service lines within the organization. This position will also initiate and develop strong interpersonal relationships with both internal CSV staff and external business partners, including health plans, to facilitate successful health outcomes. As part of a safety net organization, this position will help CSV achieve the Strategic Plan Quality Pillar by focusing on patient-centered whole-person care through quality health outcomes and population health management.

Essential Functions:

  • Supervise Quality Care Coordinators (QCC) and Transition of Care (TOC) Coordinators in Kern/Fresno County, as assigned.

  • Select, onboard, supervise, and manage billing staff, including scheduling, time off coordination and approvals, timecard maintenance and approvals, staff appraisals, and disciplinary actions.

  • Perform introductory and annual employee review competency forms and provide constructive feedback on their performance to help meet organizational and professional goals.

  • Flexibility to alter the 8:00 am - 5:00 pm schedule when necessary to meet department/business needs.

  • Perform QCC duties as needed to meet the department/business needs of the organization.

  • Travel to clinic sites in both Kern and Fresno

  • Meet with internal and external stakeholders.

  • Participate in PHMI Care Teams.

  • Formulate and propose recommended solutions/options to address CSV population and programmatic challenges in the safety net, with moderate guidance.

  • Under the direction of Quality Leadership, assume ownership for successful development, planning, and execution of projects, from the initial concept to implementation stage.

  • In collaboration with the Quality Leadership, implement PDSAs for quality improvement initiatives.

  • With moderate guidance and supervision from Quality Leadership, develop a comprehensive analysis of department performance data concerning trends observed over time.

  • Leaders will operate within the guidelines of State and Federal Employment laws, where applicable.

You'll be successful with the following qualifications:

  • Bachelor's degree in public health or related field. An equivalent combination of education and experience may be considered.

  • Minimum of 5 years of work experience in health care or a related field, doing problem-solving, policy, and/or project implementation.

  • Certification as a Community Health Worker (CHW) preferred.

  • Must be knowledgeable in HEDIS measures, UDS clinical measures, and the PCMH model of care.

  • Experience building relationships with organizations and business partners

  • At least two years of experience leading project teams and supervising.

  • Medical Assistant background preferred.

  • Demonstrated critical thinking, problem-solving, project planning, and implementation skills.

  • Demonstrated strong writing and communication skills.

  • Demonstrated ability to synthesize and analyze qualitative and/or quantitative data.
