POSITION SUMMARY

Assesses, plans, implements, and evaluates patient and family/caregiver needs with respect to patient's psychosocial health. Provides individual, family, and/or group therapy, creates and supervises an interdisciplinary rehabilitation plan.

PRINCIPLE DUTIES AND RESPONSIBILITIES

  • Assesses patients' mental health status; assesses needs related to life skills, including family, social, and work relationships

  • Gathers histories of mental/physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts; identifies precipitating events

  • Performs psychosocial evaluations on mental health patients presenting in the Emergency Department

  • Available for psychosocial consults on patients admitted to medical/surgical floors

  • Develops comprehensive life skills/diversionary program to promote mental social, physical and spiritual well-being, incorporating cognitive behavioral and Dialectical Behavioral Therapy (DBT)

  • Conducts 1:1 and group therapy sessions, as indicated

  • Provides patient/family and/or caregiver education

  • Participates in interdisciplinary care planning and discharge planning

  • Documents patient care, including assessments and care plans, DSM diagnosis, progress notes, discharge summaries/ recommendations for discharge/future care

  • Participates in utilization review activities

  • Collaborates with other health care providers, professionals, and community partners (e.g. Coos County Mental Health) to address patients' needs for care, personal growth, and wellness

  • Maintains regular, consistent and punctual attendance at the assigned job location

  • Strong group skills

  • Knowledge and understanding of cognitive behavioral and Dialectical Behavioral Therapy theories, principles, and practices

  • Effective problem solving skills

  • Effective teaching skills

  • Knowledge of community resources and how to access

  • Able to integrate care from a variety of sources to meet patient's needs

  • Able to work flexible schedule

EDUCATION/CERTIFICATIONS/LICENSES/DEGREES

  • Meets the criteria for practicing as a Qualified Mental Health Professional in the state of Oregon as one of the following:

  • Psychiatrist or physician, licensed to practice in the State of Oregon

  • Individual with a graduate degree in psychology, social work, or other mental health-related field

  • Registered nurse with a graduate degree in psychiatric nursing, licensed in the State of Oregon

  • Registered nurse with a Bachelor's degree and demonstrable skills in the Qualified Mental Health Professional role

  • An individual with registration as an occupational therapist

  • An individual with a graduate degree in recreational therapy

  • Ability to read, write legibly, and converse effectively in the English language

  • American Heart Association Healthcare Provider Basic Life Support (BLS) or must be obtained by the last day of the month after the 60 day grace period

  • Non-violent crisis intervention certification or obtain within 6 months of hire

  • Therapeutic Recreational Specialist certified by the National Council for Therapeutic Recreation preferred

EXPERIENCE

  • At least 3 years experience in field of psychiatric patient care
