Qualified Intellectual Disabilities Professional-Qidp/Case Manager

Job Summary:

Ray Graham Association is seeking a Qualified Intellectual Disabilities Professional (QIDP) to serve as a Case Manager. The QIDP will be responsible for coordinating and overseeing the delivery of services to individuals with Intellectual and Developmental Disabilities, focusing on achieving their personal goals and aspirations.

Responsibilities:

• Act as the central point of contact for families, guardians, and agencies involved in the care of individuals with disabilities.

• Establish and nurture relationships to facilitate the provision of necessary services.

• Collaborate with healthcare providers to ensure the highest quality of care.

• Conduct and assess evaluations to determine individual outcomes.

• Develop personalized plans and support individuals in pursuing their objectives and aspirations.

Qualifications:

• Bachelor's degree in a Human Service field as required by the Illinois Department of Human Services.

• Minimum of one year of direct experience working with individuals with developmental disabilities.

• Strong verbal and written communication skills.

• Clear State Police history and Health Care Worker Registry background check.

• Possession of a valid Illinois driver's license.

• Successful completion of a pre-employment drug screening.

• Ability to work independently and collaboratively in a fast-paced environment.

Starting Salary:
