As a Qualified Developmental Disability Professional/Case Manager your role will be to establish supports and develop programs to help people with disabilities achieve their goals and dreams. Facilitate team meetings, track and trend program data. Provide daily oversight to your caseload while continuously building relationships with the people supported and guardians to assure they are receiving the highest in quality services.

Essential Key Job Responsibilities

State & Federal Regulations

Implements the Overall Service Plans, Personal Outcome Measures, Waiver Survey guidelines, Center for Medicare and Medicaid (CMS) guideline and any other organization regulations or standards.

Coordinates services for people supported.

  • Identifies and establishes supports needed to assist each person to achieve his/her goals and dreams.

  • Facilitates in the development, implementation and monitoring of person centered service plans and overall service plans. This includes appropriate assessments.

  • Facilitates interdepartmental team meetings.

  • Implements and completes Therap documentation including GER's, T-logs, scom's and modules as assigned.

  • Ensures staff are trained and competent to implement programmatic strategies.

  • Ensures appropriate documentation is maintained for all programmatic strategies. This also includes daily, weekly or monthly review and analysis of all programmatic data as assigned.

  • Assists people to maintain their rights, their personal health needs and all other personal outcome measures.

  • Advocates with employers, families and other stakeholders to develop needed supports and services for the people we support.

  • Maintains benefits and other financial matters for each person on their caseload. This would include Medicare, Medicaid, Social Security, burial, etc.

  • Completes on-site program coaching and observations weekly to ensure supports are in place for each person.

  • Ensures frequent communication with each person on their caseload and family members as requested.

  • Completes Supports Intensity Scale (SIS)

Coordinates departmental enhancement.

  • Solicits new referrals and participates in referral and new admissions processes.

  • Implements all state-regulated expectations within the department.

  • Participates in the agency on-call system.

Oversees employment activities.

Ensures meaningful Vocational Services for each person on their caseload.

  • Bachelor's degree in human services, education or related field.

  • At least two years professional experience working in the field of developmental disabilities.

  • At least two years supervisory experience.

  • Current valid driver's license.

  • Ability to organize and calculate data/information and maintain records according to standards.

  • Ability to plan, organize and facilitate effective communication, including effectively communicating through clear speaking and writing.

Duties and responsibilities include working with individuals with developmental disabilities who apply for services and helping through the application process, assessing the functional level to determine substantial limitations and working with them to determine eligibility for services and the desired outcomes of the services. The case manager will gather all pertinent information to develop an interdisciplinary team and write an individualized service plan. The case manager will monitor the plan through the Quality Enhancement Review process, do regular assessments, complete face to face visits with clients and guardians and maintain an up-to-date client electronic record. Position requires some travel and evenings. Preference will be given to those with professional job experience that includes Case Management or a professional level role serving individuals with developmental disabilities. Minimum Qualifications Must have one year of experience as a Developmental Disabilities Case Manager I in the ND Department of Human Services or meet the following definition of a Qualified Developmental Disabilities Professional (QDDP): "A person who has at least one year of direct care experience working with persons with a mental illness or a developmental disability; AND has a bachelor's or master's degree in one of the following fields: social work, psychology, counseling, nursing, occupational therapy, physical therapy, child development and family science, communication disorders (includes audiologist or speech pathology), severely multiply handicapped, special education, vocational rehabilitation, sociology, elementary education, recreation therapy, or human resource management (human services track). (Certification or licensure in one of the above fields is not required for a QDDP designation)." Further requirements include valid driver's license and background check.
